摘 要:目的:探讨左西孟旦预处理在减轻大鼠在体肺缺血再灌注损伤(LIRI)的作用及其机制。方法:建立大鼠在体LIRI模型,将50只SD大鼠(购自安徽医科大学动物实验中心)随机均分为5组,假手术组(Sham组)、缺血再灌注损伤组(I/R组)、缺血预处理组(IPC组)、左西孟旦预处理组(Levo组)、IPC+Levo组,检测各组动脉血气结果、计算呼吸指数(RI),检测肺组织中丙二醛(MDA)含量、超氧化物歧化酶(SOD)活性,肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-10的表达,湿/干比值(W/D)并观察肺组织的病理形态学变化。不同组间比较采用单因素方差分析,组间两两比较采用Tukey检验。结果:与Sham组比较,I/R组RI值显著升高,肺组织MDA含量明显增加、SOD活性显著降低,TNF-α、IL-6、IL-10表达升高,W/D明显升高[RI:0.65±0.15比0.29±0.05;MDA:(8.42±0.44)nmol/mg比(2.40±0.12)nmol/mg;SOD:(5.86±0.91)U/mg比(25.93±1.42)U/mg;TNF-α:(253.83±6.11)pg/ml比(68.59±1.63)pg/ml;IL-6:(4.99±0.35)pg/ml比(1.68±0.16)pg/ml;IL-10:(29.75±5.51)pg/ml比(18.06±3.51)pg/ml;W/D:6.66±0.47比5.08±0.25,P<0.05];与I/R组比较IPC组和Levo组RI值显著降低,肺组织MDA含量明显减少、SOD活性显著增高,TNF-α、IL-6表达降低,IL-10水平升高,W/D明显降低(F=50.741、584.433、493.650、187.000、322.733、52.176、42.87,P<0.05);IPC组各指标与Levo组比较差异无统计学意义;与IPC组比较IPC+Levo组RI值明显降低,肺组织MDA含量明显减少、SOD活性显著增高,TNF-α、IL-6表达降低,IL-10水平升高,W/D明显降低[RI:0.36±0.08比0.45±0.12;MDA:(4.32±0.31)nmol/mg比(6.41±0.37)nmol/mg;SOD:(12.56±1.54)U/mg比(9.66±1.25)U/mg;TNF-α:(143.42±6.23)pg/ml比(154.00±5.76)pg/ml;IL-6:(2.47±0.14)pg/ml比(3.13±0.24)pg/ml;IL-10:(48.54±7.74)pg/ml比(43.79±6.72)pg/ml;W/D:5.31±0.13比5.78±0.32,P<0.05]。结论:运用左西孟旦预处理后可以减轻LIRI,其肺保护的机制可能与减轻脂质过氧化反应及炎性细胞的浸润有关。Objective To explore the protective role of Levosimendan precondition against lung ischemia reperfusion injury(LIRI)in rat model.Method LIRI model was established.A total of 50 SD rats were divided into 5 groups at random,Sham group,Ischemia reperfusion injury group(I/R),Ischemia precondition group(IPC)and Levosimendan precondition group(Levo),IPC+Levo group.Arterial blood gas results were detected and respiratory index(RI)was calculated in each group.The level of malondialdehyde(MDA),the activity of Superoxide Dismutase(SOD),the expressions of tumor necrosis factor-α(TNF-α),interleukin(IL)-6 and IL-10,the ratio of W/D were tested and lung pathology changes were observed in all groups.Results Compared with Sham group,the level of RI,MDA,the expressions of TNF-α,IL-6 and IL-10 and the ratio of W/D were higher significantly,while,the activity of SOD was lower significantly in I/R group[RI:0.65±0.15 vs.0.29±0.05;MDA:(8.42±0.44)nmol/mg vs.(2.40±0.12)nmol/mg;SOD:(5.86±0.91)U/mg vs.(25.93±1.42)U/mg;TNF-α:(253.83±6.11)pg/ml vs.(68.59±1.63)pg/ml;IL-6:(4.99±0.35)pg/ml vs.(1.68±0.16)pg/ml;IL-10:(29.75±5.51)pg/ml vs.(18.06±3.51)pg/ml;W/D:6.66±0.47 vs.5.08±0.25,P all<0.05].Compared with I/R group,the level of RI,MDA,the expressions of TNF-αand IL-6 as well as the ratio of W/D were lower significantly,while,the activity of SOD and the expression of IL-10 were higher significantly(F=50.741,584.433,493.650,187.000,322.733,52.176,42.87,P<0.05)in both IPC and Levo group.There were no significant differences between IPC and Levo group.Compared with IPC group the level of RI,MDA,the expressions of TNF-αand IL-6 as well as the ratio of W/D were lower significantly,while,the activity of SOD and the expression of IL-10 were higher significantly[RI:0.36±0.08 vs.0.45±0.12;MDA:(4.32±0.31)nmol/mg vs.(6.41±0.37)nmol/mg;SOD:(12.56±1.54)U/mg vs.(9.66±1.25)U/mg;TNF-α:(143.42±6.23)pg/ml vs.(154.00±5.76)pg/ml;IL-6:(2.47±0.14)pg/ml vs.(3.13±0.24)pg/ml;IL-10:(48.54±7.74)pg/ml vs.(43.79±6.72)pg/ml;W/D:5.31±0.13 vs
