调脉合剂干预气阴两虚室上性心律失常随机平行对照研究

摘　　要：目的:观察调脉合剂干预气阴两虚室上性心律失常(心脏自主神经功能异常)的疗效。方法:使用随机平行对照方法,将90例气阴两虚室上性心律失常患者随机分为对照组和治疗组,各45例。两组患者均予以基础常规治疗,对照组采用酒石酸美托洛尔片治疗,治疗组采用调脉合剂治疗,均连续治疗4周。对比两组患者中医证候疗效、心率变异性(HRV)、24 h平均心率及不良反应。结果:治疗组总有效率为95.56%(43/45),对照组总有效率为73.33%(33/45),治疗组优于对照组(P<0.05);两组患者治疗后心率变异性指标全程正常窦性心博NN间期的标准差(SDNN)、全部相邻NN间期之差的均方根(rMSSD)、≤0.40Hz的所有NN间期的变异(TP)、0.15~0.40 Hz范围内NN间期的变异(HF)均较治疗前升高(P<0.05),治疗组改善优于对照组(P<0.05),0.04~0.15 Hz范围内NN间期的变异(LF)较治疗前均无明显变化(P>0.05);两组患者治疗后24 h平均心率均较治疗前降低(P<0.05),组间无明显差异(P>0.05);两组均无严重不良反应。结论:调脉合剂治疗气阴两虚室上性心律失常(心脏自主神经功能异常)疗效满意,可更好调整患者心脏自主神经功能,无严重不良反应。Objective:To observe the therapeutic effect of Tiaomai Mixture in the treatment of supraventricular arrhythmias(cardiac autonomic nerve dysfunction)with Qi and Yin Deficiency.Methods:Based on random parallel control method,90 patients were randomly divided into two groups,45 patients in each.Both groups received basic routine treatment.The patients were treated by metoprolol tartrate tablets in control group and Tiaomai Mixture in treatment group.Continuous treatment for four weeks is a course of treatment.TCM syndrome score,heart rate variability(HRV)index,24-hour average heart rate and adverse reactions were observed.Results:The total effective rate was 95.56%(43/45)in treatment group and 73.33%(33/45)in control group.The treatment group was superior to the control group(P<0.05).Heart rate variability indexes,SDNN,rMSSD,TP and HF were improved in both two groups,and the treatment group showed better improvement than control group(P<0.05).There was no significant difference in LF between before and after treatment(P>0.05).The 24-hour average heart rate decreased significantly in both two groups(P<0.05),and there was no significant difference between the two groups(P>0.05).There were no serious adverse reactions in both groups.Conclusion:Tiaomai Mixture has a satisfactory effect in treating supraventricular arrhythmia(cardiac autonomic nervous dysfunction)due to Qi and Yin Deficiency.It can better adjust the cardiac autonomic nervous function of patients without serious adverse reactions.
