摘  要:目的探讨实时剪切波弹性成像技术(SWE)在胎盘弹性中的应用价值。方法选择2017年6月-2019年1月在我院产前检查并分娩的孕妇80例(胎盘为前壁),孕周27-38周,采用实时剪切波弹性成像超声诊断仪进行胎盘弹性值的测量,每个胎盘区域测量3次,取平均值。随后记录所有受试对象的妊娠结局,观察胎儿预后不良的情况。并对预后不良胎儿的胎盘进行病理研究,判断胎盘的病理改变程度。根据妊娠结局将其分为胎儿预后不良组、正常组,比较两组之间胎盘弹性值的差异。结果胎儿预后不良组37例胎盘弹性均值28kPa(17.8-36kPa),正常组43例胎盘弹性均值6kPa(4.78-7.85kPa),两组差异有统计学意义(P<0.05)。结论SWE可用于胎盘硬度的定量评估,能够作为胎盘功能监测的重要手段,从而为临床围产期保健及改善围产儿结局方面提供一定的帮助。Objective To explore the application value of real-time shear wave elastography(SWE)in placental elasticity.Methods From June 2017to January 2019,80cases of pregnant women(with the placenta as the anterior wall)were selected with gestation weeks of 27-38.The real-time shear wave elastography ultrasonic diagnostic instrument was used to measure the placenta elasticity value,and the mean value was taken as the data for the final statistical analysis.The pregnancy outcomes of all subjects were then recorded and the poor prognosis of the fetus was observed.The placenta of fetuses with poor prognosis was studied to determine the pathological changes of the placenta.According to the outcome of pregnancy,they were divided into the group with poor fetal prognosis and the normal group,and the differences of placental elasticity between the two groups were compared.Results The mean placental elasticity of 37cases in the group with poor fetal prognosis was 28kPa(17.8-36kPa),while that of 43cases in the normal group was 6kPa(4.78-7.85kPa).The latter is significantly lower than the former(P<0.05).Conclusion SWE can be used for quantitative assessment of placenta hardness and can be used as an important means of monitoring placenta function,thus providing certain help for clinical perinatal health care and improving perinatal outcome.
