摘 要:目的分析血清白介素-12(IL-12)与血清淀粉酶样蛋白A(SAA)在新生儿感染性疾病中的表达水平及其对新生儿感染性疾病的诊断效果。方法回顾性分析医院收治的82例新生儿感染性疾病患儿的病例资料(作为病例组),选取同时期健康新生儿50例作为对照组。通过双抗免疫夹心法检测血清IL-12水平,酶联免疫吸附试验法检测血清SAA水平。比较2组对象血清IL-12、SAA水平,并通过ROC曲线评价IL-12、SAA对新生儿早期感染的诊断效能。结果病例组血清IL-12、SAA水平显著高于对照组,差异均有统计学意义(P<0.05)。治疗1周后,病例组患儿血清IL-12、SAA水平显著低于治疗前,差异均有统计学意义(P<0.05)。细菌感染患儿血清IL-12水平显著高于病毒感染患儿,血清SAA水平显著低于病毒感染患儿,差异均有统计学意义(P<0.05)。以SAA≥17.95mg/L为界早期诊断新生儿感染性疾病的敏感度和特异度分别为88.18%和90.67%,以IL-12≥283.58mg/L为界早期诊断新生儿感染性疾病的敏感度和特异度分别为80.34%和72.05%,IL-12与SAA联合诊断的敏感度和特异度为96.49%和70.92%。结论血清IL-12和SAA水平在新生儿感染性疾病疗效评估及细菌、病毒感染鉴别中均具有较好的应用效果,且联合检测IL-12和SAA水平对新生儿感染性疾病早期诊断具有重要临床价值,值得临床重视。Objective To analyze the expression level of Serum Interleukin-12(IL-12)and Serum amyloid-like protein A(SAA)in neonatal infectious diseases and its diagnostic effect on neonatal infectious diseases.Methods 82cases of neonatal infectious diseases in hospital were retrospectively analyzed,and divided into severe infection group(39cases)and mild infection group(43cases)according to the patient's condition,50healthy newborns in the same period were selected as the control group.Serum IL-12level was detected by double antibody sandwich method,serum SAA level was detected by enzyme-linked immunosorbent assay.The serum levels of IL-12and SAA in three groups were compared,the value of serum IL-12and SAA in judging the progression of children to severe infectious diseases was evaluated by ROC curve.Results The serum levels of IL-12and SAA in the case group were significantly higher than those in the control group(P<0.05).After 1week of treatment,the serum levels of IL-12and SAA in the case group were significantly lower than those before treatment(P<0.05).When SAA≥17.95mg/L was used as the positive criterion,the sensitivity and specificity in diagnosis of neonatal infectious diseases were 88.18%and 90.67%,when IL-12≥283.58ng/L was used as the positive criterion,the sensitivity and specificity in diagnosis of neonatal infectious diseases were 80.34%and 72.05%,when using SAA≥17.95mg/L and IL-12≥283.58ng/L as the combinatorial positive criteria,the sensitivity and specificity in diagnosis of neonatal infectious diseases were 96.49%and 70.92%.Conclusion The serum levels of IL-12and SAA have a good application effect in the efficacy evaluation and the differential diagnosis of bacterial and viral infections,and the combined detection of IL-12and SAA has an important clinical value inearly diagnosis of neonatal infectious diseases,It is worthy of clinical attention.
