摘　　要：将国家最新的养老服务精神与老年人对健康管理需求相结合,充分发挥城市智慧医疗技术,进一步优化城市社区老年人健康管理模式。通过文献分析、数据分析、随机访谈来访者等方法,找准老年人群体对健康管理的真正需求,解决目前城市社区老年人健康管理模式存在着供需不匹配、医疗服务差、服务路径少等痛点、难点。利用国家卫生健康委员会的最新政策优势,利用现代智慧医疗技术优势,从顶层设计、合法经营、多元模式、智慧养老等方面进行社区老年人健康管理模式的优化升级,打造家庭、社区、医院多元联动的城市社区老年人健康管理智慧医疗新模式。By combining the latest national spirit of old-age service with the elderly’s demand for health management,and giving full play to urban smart medical technology,the health management model of the elderly in urban communities will be further optimized.Through literature analysis,data analysis,random interviews and other methods,the real needs of the elderly population for health management were identified,and the pain points and difficulties such as mismatch between supply and demand,poor medical service,and few service paths existed in the current health management model for the elderly in urban communities were solved.Take advantage of the latest of the health committee of the national health policy,take advantage of modern medical technology wisdom,from the top design,legitimate business,multiple patterns,the wisdom pension community elderly health management model optimization,makes the family,community,hospital many ways of urban community elderly health management wisdom medical new patterns.
