摘 要:目的观察经电针治疗后神经源性膀胱(NB)大鼠尿动力学变化,并观察Cajal间质细胞(ICC)及表面P2X5表达的变化以探讨该电针疗法的作用机制。方法雌性SD大鼠随机分为5组:正常组、假手术组、模型组、电针关元组和电针次髎组,通过重物坠落法建立脊髓损伤(SCI)模型。电针组每日给予电针处理,持续14d。通过分析最大容量及膀胱顺应性评估尿流动力学变化,并通过免疫荧光分析ICC和P2X5的表达变化。结果在膀胱最大容量方面,模型组明显高于正常组(P<0.01);电针关元组和电针次髎组均明显低于模型组(P<0.01),但均高于正常组(P<0.01);电针次髎组明显低于电针关元组(P<0.01)。在膀胱顺应性方面,结果与膀胱最大容量基本一致。ICC细胞及其受体P2X5阳性表达率,模型组低于正常组(P<0.01);电针关元组和电针次髎组均明显高于模型组(P<0.01),但均低于正常组(P<0.01);电针次髎组明显高于电针关元组(P<0.01)。结论对于SCI引起的膀胱功能障碍,电针次髎穴可显著改善膀胱顺应性,其机制可能是通过上调ICC及其表面受体P2X5的表达,改善逼尿肌收缩性。Objective To observe the urodynamic changes of neurogenic bladder(NB)rats after electro-acupuncture(EA),and to observe the changes of the expression of interstitial cells of Cajal(ICC)and P2 X5 on the surface to explore the mechanism of EA.Methods 50 female SD rats were randomly divided into 5 groups,including normal group,sham operation group,model group,EA at Guanyuan acupoint group and EA at Ciliao group.The spinal cord injury(SCI)model was established by weight falling method.EA group was given EA treatment daily for 14 d.The urodynamic changes were evaluated by analyzing the maximum volume and bladder compliance,and the expression of ICC and P2 X5 were analyzed by immunofluorescence.Results In the aspect of maximum bladder volume,model group was significantly higher than normal group.For ICC cells and its receptor P2 X5 positive expression rate,model group was lower than normal group(P<0.01);EA at Guanyuan acupoint group and EA at Ciliao acupoint group were significantly higher than model group(P<0.01),but lower than normal group(P<0.01);EA at Ciliao acupoint group was significantly higher than EA at Guanyuan acupoint group(P<0.01).Conclusion For the bladder dysfunction caused by SCI,EA at acupoint Ciliao can significantly improve the bladder compliance.The mechanism may be that it can improve the detrusor contractility by up regulating the expression of ICC and P2 X5.
