芪黄汤联合糖皮质激素对血小板减少性紫癜患者血小板相关抗体、血小板生成素及炎症因子的影响

摘　　要：目的观察芪黄汤联合糖皮质激素对血小板减少性紫癜(ITP)患者血小板相关抗体、血小板生成素及炎症因子的影响。方法选取我院86例ITP患者作为研究对象,分为观察组、对照组各43例。对照组采用糖皮质激素进行治疗,观察组采用芪黄汤联合糖皮质激素进行治疗,比较两组的总有效率、血小板相关抗体(PAIgA、PAIgG)、血小板生成素(TPO)、白细胞介素(IL-6、IL-10)及肿瘤坏死因子-α(TNF-α)的水平。结果观察组总有效率88.3%(38/43)高于对照组60.5%(26/43)(P<0.05)。治疗后两组PAIgA、PAIgG、TPO均下降(P<0.05),且观察组PAIgA、PAIgG、TPO均低于对照组(P<0.05)。治疗后两组IL-6、TNF-α均下降,IL-10增高(P<0.05);且观察组IL-6、TNF-α均低于对照组,IL-1高于对照组(P<0.05)。结论芪黄汤联合糖皮质激素明显提高ITP患者的疗效,降低血小板相关抗体、炎症因子的表达。Objective To observe the effects of Qihuang Decoction combined with glucocorticoids on platelet-associated antibodies(PAIgA,PAIgG),thrombopoietin(TPO),interleukin-6(IL-6),IL-10 and tumor necrosis factor-α(TNF-α)in patients with thrombocytopenic purpura(ITP).Methods 86 cases of patients with ITP were divided into observation group and control group,with 43 cases each.Control group was treated with glucocorticoids,observation group was treated with Qihuang Decoction combined with glucocorticoids.The efficacy of the two groups PAIgA PAIgG,TPO,and IL-6,IL-10 and TNF-αwere compared.Results The total effective rate in observation group was 88.3%(38/43),higher than that in control group 60.5%(26/43)(P<0.05).PAIgA PAIgG,and TPO in both groups decreased after treatment(P<0.05);PAIgA PAIgG,and TPO in observation group were lower than those in control group after treatment(P<0.05).After treatment,the levels of IL-6,TNF-αwere decreased in both groups,and IL-10 was increased(P<0.05);the levels of IL-6 and TNF-αin observation group were lower than those in control group,and IL-1 was higher than that in control group(P<0.05).Conclusion Qihuang Decoction combined with glucocorticoid significantly improves the efficacy of ITP,and reduces the expression of platelet-related antibodies and inflammatory factors.
