清热解毒软胶囊联合多索茶碱对慢阻肺患者免疫功能及动脉血气指标的影响

摘　　要：目的观察清热解毒软胶囊辅助多索茶碱治疗对慢阻肺(COPD)患者免疫功能及动脉血气指标的影响。方法选择我院收治的112例COPD患者,随机分为对照组与观察组各56例,对照组给予多索茶碱治疗,观察组在对照组治疗基础上给予清热解毒软胶囊辅助治疗。统计两组患者临床疗效;测评两组患者治疗前、后的用力肺活量(FVC)、深吸气量(IC)以及1秒用力呼气容积(FEV1)/FVC的比(FEV1%)肺功能相关指标;比较两组患者治疗前、后的动脉血氧饱和度(SaO2)、血氧分压(PaO2)以及血二氧化碳分压(PaCO2)血气指标水平变化;比较两组患者治疗前、后的免疫球蛋白A(IgA)、免疫球蛋白G(IgG)以及免疫球蛋白M(IgM)水平变化;统计两组患者的不良反应总发生率。结果治疗后,观察组与对照组总有效率比较差异无统计学意义(P>0.05);观察组FVC、IC和FEV1%3项肺功能指标均高于对照组(P<0.05);观察组SaO2与PaO2水平均高于对照组(P<0.05),PaCO2水平低于对照组(P<0.05);观察组IgA、IgG与IgM三项免疫功能指标均高于对照组(P<0.05);观察组不良反应总发生率低于对照组(P<0.05)。结论清热解毒软胶囊辅助多索茶碱治疗COPD有利于肺功能、血气水平和免疫功能的恢复,不良反应较少。Objective To observe the effects of Qingre Jiedu Soft Capsule assisted doxofylline on immune function and arterial blood gas indexes in patients with chronic obstructive pulmonary disease(COPD).Methods 112 COPD patients were randomly divided into control group and observation group,56 cases in each group.Control group was treated with doxofylline,while observation group was treated with Qingre Jiedu Soft Capsule on the basis of control group.The clinical curative effect in both groups was statistically analyzed.The pulmonary function related indexes such as forced vital capacity(FVC),inspiratory capacity(IC)and forced expiratory volume in 1 second(FEV1)/FVC(FEV1%)before and after treatment in both groups were measured.The changes in levels of blood gas indexes such as arterial blood oxygen saturation(SaO2),partial pressure of blood oxygen(PaO2)and partial pressure of carbon dioxide(PaCO2)before and after treatment were compared between the two groups;the changes in levels of immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)before and after treatment were compared between the two groups;the total incidence of adverse reactions in both groups was statistically analyzed.Results There was no significant difference in total response rate of treatment between the two group(P>0.05);FVC,IC and FEV1%in observation group were higher than those in control group(P<0.05);levels of SaO2 and PaO2 in observation group were higher than those in control group(P<0.05),while PaCO2 level was lower than that in control group(P<0.05);IgA,IgG and IgM in observation group were higher than those in control group(P<0.05);the total incidence of adverse reactions in observation group was lower than that in control group(P<0.05).Conclusion Qingre Jiedu Soft Capsule assisted doxofylline is beneficial to recovery of pulmonary function,blood gas level and immune function,with fewer adverse reactions in treatment of COPD.
