坤灵丸联合低分子肝素在体外授精-胚胎移植中应用的临床研究

摘　　要：目的观察坤灵丸联合低分子肝素对体外受精-胚胎移植(IVF-ET)早期妊娠结局的影响。方法将93例患者分为治疗组与对照组,治疗组移植前2个月和移植当月口服坤灵丸至移植后12d。移植后12~14d确定妊娠之日开始皮下注射低分子肝素钙,两组术后均常规黄体支持。监测移植后D-二聚体和子宫动脉血流阻力指数和,比较两组的早期妊娠结局。结果两组移植后D-二聚体有统计学差异(P<0.05),各组水平间无统计学差异(P>0.05)。两组移植后子宫动脉血流阻力指数无统计学差异(P>0.05),组内水平之间有非常显著的统计学差异(P<0.01)。两组流产率分别为4.44%(2/45)、18.75%(9/48),可持续妊娠率分别为95.55%(43/45)和81.25%(39/48),二组比较有统计学差异(P<0.05)。结论坤灵丸联合低分子肝素,降低IVF-ET早期流产率。Objective To observe the effect of Kunling Pills combined with low molecular weight heparin on early pregnancy outcome after in vitro fertilization-embryo transfer(IVF-ET).Methods 93 patients were divided into treatment group and control group,treatment group took oral Kunling Pills 2 months before and 12 d after transplantation.Pregnancy were confirmed 12-14 d after transplantation,treatment group was given hypodermic injected LMWH on the date of pregnancy both groups were given rontine luteal support.D-dimer and uterine artery blood flow resistance index were monitored in both groups.The early pregnancy outcomes of IVF-ET were compared between the two groups.Results There were statistically significant differences(P<0.05)in D-dimer between the two groups,and there were no statistical difference between the levels of the two groups(P>0.05);there were no statistically significant difference(P>0.05)in uterine arterial blood between the two groups,but there were significant statistical differences between the levels within the group(P<0.01);the miscarriage rates and the sustainable pregnancy rates and ongoing prengnancy rate in treatment group and control group were 4.44%(2/45),18.75%(9/48),95.55%(43/45)and 81.25%(39/48),respectively,there were statistically significant differences between the two groups(P<0.05).Conclusion Kunling Pills combined with low molecular weight heparin can reduce early miscarriage rate of IVF-ET.
