摘　　要：产后出血(postpartum hemorrhage,PPH)是世界范围内孕产妇死亡的首位原因,缩宫素是预防和治疗产后出血的一线药物,但子宫平滑肌上的缩宫素受体(oxytocin receptor,OTR)在某种情况下会发生脱敏感,导致子宫对缩宫素的反应性下降。目前临床缺乏公认的OTR脱敏的预防和处理指南,结合现有的证据,本文就缩宫素的作用机制、OTR的脱敏和复敏、识别OTR脱敏的高危因素、预防OTR脱敏以及OTR脱敏后的药物处理几个方面的研究进展进行综述。Postpartum hemorrhage(PPH)was the primary cause of maternal mortality worldwide.Oxytocin was the first line drug in the prevention and treatment of PPH.Whereas the oxytocin receptor(OTR)in the myometrium may desensitize in some situations,which further attenuated the reaction of uterine to oxytocin.There was no recognized guideline for the prevention and treatment of OTR desensitization currently.Combined with the available evidence,this review aimed to illuminate the research progress on the mechanism of the action of oxytocin,the desensitization and resensitization of OTR,identifying high-risk factors of the desensitization of the OTR,as well as the prevention and medicament treatment of OTR desensitization.
