放散式体外冲击波压痛点结合穴位治疗老年髋骨关节炎临床研究

摘　　要：目的:研究放散式体外冲击波压痛点结合穴位治疗老年髋骨关节炎对患者髋关节功能、炎症因子水平的影响。方法:选取老年髋骨关节炎患者59例按随机数字表法分组,对照组29例行放散式体外冲击波局部压痛点治疗,治疗组30例在局部压痛点治疗的基础上结合相关穴位进行治疗,均连续治疗8周。分别采用骨性关节炎指数量表(WOMAC)、6 min步行试验(6MWT)于治疗前、治疗结束时、治疗后3个月时以疼痛积分和功能积分评价变化情况;分别于治疗前、治疗结束当天采集静脉血液ELISA法测定血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)水平情况。结果:与治疗前比较,治疗结束时、3个月后两组患者的WOMAC疼痛评分、功能评分均显著改善,治疗组两项评分降幅优于对照组(P<0.05);治疗结束时、3个月后治疗组患者6MWT增幅显著高于对照组(P<0.05);治疗结束时治疗组TNF-α、IL-1β水平显著低于对照组(P<0.05)。结论:放散式体外冲击波压痛点结合穴位治疗老年髋骨关节炎可更好地缓解关节疼痛,恢复下肢运动能力,降低机体炎症因子。Objective:To investigate the effect of release-type extracorporeal shock wave tenderness points combined with acupoint theraphy on hip joint function and inflammatory factors in elderly patients with hip osteoarthritis.Methods:A total of 59 elderly patients with hip osteoarthritis were randomly divided into two groups.The control group(29 cases)was treated with release-type extracorporeal shock wave local tenderness points.The treatment group(30 cases)was combined with acupoints therapy on the basis of release-type extracorporeal shock wave tenderness points treatment.Both groups were treated for 8 weeks.The osteoarthritis index scale(WOMAC),6 minutes walking test(6 MWT)before treatment,at the end of treatment,3 months after treatment were used to evaluate the changes with pain score and functional score respectively.The serum levels of tumor necrosis factor-α(TNF-α)and interleukin-1βin venous blood were detected ELISA before and at the end of treatment.Results:Compared with before treatment,the WOMAC pain score and functional score of the two groups were significantly improved at the end of treatment and 3 months after treatment.The scores of the treatment group were lower than those of the control group(P<0.05).At the end of treatment and 3 months after treatment,the 6MWT increase in the treatment group was significantly higher than that in the control group(P<0.05).At the end of the treatment,the levels of TNF-αand IL-1βin the treatment group were significantly lower than those in the control group(P<0.05).Conclusions:Release-type extracorporeal shock wave tenderness points combined with acupoints therapy can better relieve joint pain,restore lower limb exercise ability,and reduce body inflammatory factors of elderly patients with hip osteoarthritis.
