摘  要:目的研究脐动脉血血气分析和乳酸测定对新生儿预后的评估价值。方法收集2018年10月-2019年6月129例于我院分娩的新生儿,住院分娩前脐动脉血流监测正常,产前、产时胎心监测正常,足月分娩无妊娠并发症,Apgar评分均≥8分,出生后第一次啼哭前钳夹脐带抽取新生儿脐动脉血测定血气分析与乳酸。结果pH与PaCO2、LAC呈显著负相关,与PaO2呈显著正相关(P<0.05),与其他血气分析指标无显著相关性(P>0.05)。129例新生儿中,呼吸性酸中毒14例(10.85%),混合性酸中毒6例(4.65%),代谢性酸中毒3例(2.33%)。129例新生儿脐动脉血乳酸测定异常18例(13.95%),pH异常22例(16.28%),差异无统计学意义(P>0.05)。结论脐动脉血血气分析和乳酸测定能有效反映新生儿的机体状态与异常指标情况,弥补Apgar评分之不足,可为临床评估新生儿预后提供一定的依据。Objective To analyze the clinical study of neonatal prognosis in umbilical arterial blood gas analysis and lactate determination. Methods A total of 129 delivery babies in our hospital from October 2018 to June 2019 were collected. The umbilical artery blood flow monitoring was normal before hospitalization and delivery. The fetal heart monitoring was normal before and during delivery. There were no pregnancy complications during term delivery, Apgar scores all ≥8 points, Umbilical arterial blood was extracted from the newborn umbilical cord before the first cry, and blood gas analysis and lactic acid were determined.Results There was a significant negative correlation between PH and PCO2,LAC, and a significant positive correlation with PO2(P<0.05), There was no significant correlation with other blood gas analysis indicators(P>0.05);Among 129 neonates, 14 cases were respiratory acidosis(10.85%), 6 cases were mixed acidosis(4.65%), and metabolic acidosis 3 cases(2.33%). In 129 neonates, 18 cases(13.95%)had abnormal umbilical arterial blood lactate levels and 22 cases(16.28%)had abnormal PH values,The difference was not statistically significant(P>0.05).Conclusion Umbilical arterial blood gas analysis and lactate measurement in newborns can effectively reflect the newborn’s body state and abnormal indicators, can make up for the deficiency of Apgar score, and provide an effective basis for clinical assessment of newborn prognosis.
