摘　　要：目的:探讨加速康复外科(enhanced recovery after surgery,ERAS)护理在不孕症患者围手术期应用的卫生经济学评价。方法:选取2018年1月至2019年1月本院收治的拟行宫腹腔镜联合手术的女性不孕症患者412例研究对象,随机分为对照组(n=212)和ERAS组(n=200),对照组给予常规护理,ERAS组行加速康复护理干预。观察两组不孕症患者的住院时间、术前费用、手术费用、术后费用、住院总费用、卫生经济学指标。结果:ERAS组患者住院时间、术前费用、术后费用、住院总费用低于对照组(P<0.05),ERAS组术后需要医护人员时间短于对照组(P<0.05),两组患者手术费用差异无统计学意义(P>0.05)。结论:ERAS护理可缩短不孕症患者住院时间,减少医疗人力资源占用,卫生经济学效益更高,值得推广。Objective:To explore the safety and effectiveness of nursing measures based on the concept of enhanced recovery after surgery(ERAS)for infertility women undergoing hysteroscopy combined laparoscopy.Methods:A total of 412 cases of infertility patients admitted to our hospital from January 2018 to January 2019 were selected as subjects.They were divided into ERAS group(200 cases)and control group(212 cases)according to the random number table method.Patients in the ERAS group were given the nursing measures based on the concept of ERAS,and patients in the control group were given the traditional nursing measures.412 patients with infertility were enrolled with the indices of health economics evaluated.Results:The hospitalization time,the total cost in the ERAS group was lower than that of control group[(3877±1742)yuan vs(18562±2143)yuan,P<0.01].The inspection cost and the surgery cost in which two groups were no significant(P>0.05).But the postoperative cost was lower than in the control group[(2131±154)yuan vs(4345±245)yuan,P<0.01].The off work of both patients and their family members in the ERAS group[(5.73±1.31)d and(1.53±0.45)d]were shorter than those in the control group[(7.68±2.31)d and(3.63±0.76)d](P<0.01).Conclusions:Nursing measures based on the concept of ERAS can be safely applied to infertility surgery,which makes with the requirements of health economics.
