摘 要:目的探讨自我管理教育对康复期精神分裂症患者的影响。方法选取2018年7月至2019年7月无锡市精神卫生中心收治的康复期精神分裂症患者120例为观察对象,依据护理方式不同分为研究组和对照组,每组各60例。对照组给予常规护理;研究组在常规护理基础上再给予自我管理教育护理。比较干预后两组患者的用药依从性、疾病复发率、精神病评定量表(BPRS)评分、社会功能缺陷筛选量表(SDSS)评分、生活质量量表(SQLS)评分以及自我护理能力量表(ESCA)评分。结果研究组治疗后用药依从性高于对照组,疾病复发率低于对照组(均P<0.05);护理前两组BPRS、SDSS评分比较,差异无统计学意义(均P>0.05),护理后1周研究组BPRS、SDSS评分均低于对照组(均P<0.05);患者SQLS中的心理社会、动力和精力、症状和副作用3个分量表的评分于入院当天和对照组差异无统计学意义(均P>0.05),但于护理后1周和出院时均低于对照组(均P<0.05);入院时,两组患者的ESCA评分差异无统计学意义(P>0.05),护理后1周,研究组的评分高于对照组(P<0.05)。结论对康复期精神分裂症患者应用自我管理教育,提高患者用药依从性,降少疾病复发率,提高患者社会功能与自护能力,进而改善生活质量。Objective To explore the effect of self-management education on the patients with schizophrenia in rehabilitation period.Methods One hundred and twenty patients with schizophrenia in rehabilitation period treated in Wuxi Mental Health Center from July 2018 to July 2019 were selected and divided into study group and control group according to different nursing methods,with 60 cases in each group.The patients in control group were treated with routine nursing.And the patients in study group were given self-management education nursing on the basis of routine nursing.Medication compliance,disease recurrence rates,BPRS,SDSS,SQLS and ESCA scores were compared between the two groups after the intervention.Results The drug compliance of study group was higher than that of control group.And the disease recurrence rate was lower than that of control group(P<0.05).Before the intervention of nursing,there was no significant difference in BPRS and SDSS scores between the two groups(all P>0.05).And the scores of BPRS and SDSS in the study group were significantly lower than those of the control group one week after nursing significantly(all P<0.05).All the dimensions of SQLS(psychosocial,dynamic and energy,symptoms and side effects)in study group were lower than those of control group at one week after care and at discharge.At admission,there was no statistically significant difference in ESCA scale scores between the two groups(P>0.05).One week after nursing,ESCA scale score of the study group was higher than that of the control group(P<0.05).Conclusions Self-management education has a positive effect on medication compliance in patients with schizophrenia in the period of rehabilitation,which is helpful to reduce the recurrence rate of the disease and improve social function,self-care ability and the quality of life.
