消炎止痒洗剂联合丙酸氟替卡松乳膏治疗湿疹湿热浸淫型疗效观察

摘　　要：目的:观察医院中药制剂消炎止痒洗剂联合丙酸氟替卡松乳膏治疗湿疹湿热浸淫型的疗效。方法:湿疹湿热浸淫型患者98例根据入院日期的奇偶顺序分为观察组和对照组各49例。对照组给予丙酸氟替卡松乳膏外擦患处治疗,观察组在对照组基础上加用消炎止痒洗剂湿敷治疗。比较两组患者治疗前后的症状及皮损面积积分(EASI评分)、瘙痒积分(VAS评分),以及血清炎症因子水平变化,评估两组临床疗效、药品不良反应与出院后随访1个月的复发情况。结果:观察组临床总有效率为81.63%,明显高于对照组的55.10%(P<0.05)。治疗后,两组患者的EASI评分、VAS评分,以及血清白细胞介素(IL)-4、IL-10水平均较治疗前明显降低(P<0.05),且观察组各项指标均优于对照组(P<0.05)。观察组药品不良反应发生率与复发率均显著低于对照组(P<0.05)。结论:中药制剂消炎止痒洗剂联合治疗湿疹湿热浸淫型安全有效,供临床应用推广。Objective:To evaluate the curative effect of traditional Chinese medicine preparation Xiaoyan Zhiyang lotion combined with fluticasone propionate cream in the treatment of dampness and heat immersion eczema.Methods:A total of 98 patients with dampness and heat immersion eczema admitted to our hospital from August 2012 to December 2019 were selected as the study subjects.According to the odd-even sequence of admission dates,they were divided into treatment group and control group with 49 patients in each group.The control group was treated with fluticasone propionate cream for external use,and the treatment group was treated with Xiaoyan Zhiyang lotion for wet dressing additionally.The symptoms,eczema area integral score(EASI score),itching integral score(VAS score)and serum levels of inflammatory cytokines were compared between the two groups before and after treatment,and the total effective rate,adverse drug reactions and recurrence at one-month follow-up after discharge were also evaluated.Results:The total effective rate of the treatment group was 81.63%,which was higher than that of the control group(55.10%),and the difference was statistically significant(P<0.05).After treatment,EASI,VAS,IL-4 and IL-10 in the two groups were significantly reduced with significant differences when compared with those before treatment(P<0.05),and the reduction in the treatment group was more significant than that in the control group with statistically significant differences(P<0.05).The incidence of adverse drug reactions and recurrence rate in the treatment group were both significantly lower than that in the control group(P<0.05).Conclusion:It is safe and effective to treat dampness and heat immersion type eczema with Xiaoyan Zhiyang lotion combined with fluticasone propionate cream,and it is worthy of clinical application.
