循证药学在处理肿瘤双靶向治疗不良反应中的实践

摘　　要：目的:临床药师通过参与临床药物治疗,为医生和患者提供药学服务,减少药物不良反应对患者造成的损害,进而促进临床合理用药。方法:以1例肺癌患者使用西妥昔单抗联合阿法替尼致色素沉着为例,临床药师采用循证药学方法,结合临床实际情况,对不良反应发生原因及处理措施进行综合分析。结果:通过对已检索文献和患者情况分析推测,色素沉着可能是由西妥昔单抗引起,但不排除阿法替尼引起的可能。同时临床药师向医生解释具体发生原因及应对措施,并提出易引起色素沉着的抗肿瘤药物及发生机制。结论:临床药师参与临床药物治疗,为医生和患者提供药学服务,促进临床治疗方案合理化,减少不良反应对患者造成的损伤,提高患者依从性,提高临床药物治疗水平。Objective:Clinical pharmacists participate in clinical drug treatment,provide pharmaceutical services for doctors and patients,reduce the damage to patients caused by adverse drug reactions,and promote rational clinical use of drugs.Methods:Taking one case of lung cancer patient with cetuximab-afatinib induced pigmentation as an example,clinical pharmacist performed intervention through evidence-based pharmaceutical methods,and combined with clinical practice,the causes of adverse reactions and treatment measures were comprehensively analyzed.Results:Through the analysis of retrieved literature and patient’s conditions,it was speculated that pigmentation might be caused by cetuximab,while the possibility of afatinib could not be ruled out.At the same time,clinical pharmacist explained the causes and put forward the countermeasures for doctor,and provided the anti-tumor drugs and their mechanisms inducing pigmentation.Conclusion:Clinical pharmacists participate in clinical drug treatment,provide pharmaceutical services for doctors and patients,promote the rationalization of clinical treatment programs,reduce the damage caused by adverse reactions to patients,improve patients’compliance,and enhance the rational level of clinical drug treatment.
