摘　　要：目的探讨微通道经皮肾镜取石术治疗输尿管上段结石的临床疗效及预后。方法选取2017年2月至2019年7月就诊于本院的输尿管上段结石患者86例,按随机数字表法分为两组,各43例。两组均行经皮肾镜取石术治疗,其中A组经微通道(18F)取石,B组经标准通道(24F)取石。比较两组围术期指标、结石清除率以及并发症。结果A组术中出血量、术后4 h时疼痛程度评分低于B组,术后4 h时舒适度评分高于B组,差异有统计学意义(P<0.05);两组手术操作时间、术后住院时间、术后结石清除率和并发症发生率比较差异无统计学意义。结论输尿管上段结石患者使用微通道经皮肾镜取石术效果、安全性与标准通道经皮肾镜取石术相当,前者可减少术中失血量,减轻术后疼痛程度,提高患者舒适度。Objective To investigate the clinical efficacy and prognosis of microchannel percutaneous nephrolithotomy in the treatment of upper ureteral calculi.Methods 86 patients with upper ureteral calculi in our hospital from February 2017 to July 2019 were selected and divided into two groups according to the random number table method,with 43 cases in each group.Both groups were treated with percutaneous nephrolithotomy.Group A took stones via microchannel(18F)and group B took stones via standard channel(24F).The perioperative indexes,stone clearance rate and complications were compared between the two groups.Results The intraoperative blood loss and pain score at 4 hours after operation in group A were lower than those in group B,and the comfort score at 4 hours after operation was higher than that in group B,the difference was statistically significant(P<0.05).There was no statistically significant difference in post-hospital time,postoperative stone clearance rate and complication rate.Conclusion The effect and safety of microchannel percutaneous nephrolithotomy for patients with upper ureteral calculi is equivalent to standard percutaneous nephrolithotomy.The former can reduce intraoperative blood loss,reduce postoperative pain,and improve patient comfort.
