摘　　要：目的:探讨关节镜下经皮微创钢板内固定术(MIPPO)治疗胫骨平台骨折的价值。方法:选取本院2015年1月-2019年3月收治的52例胫骨平台骨折为研究对象,按照简单随机化法分为对照组[n=26,予以切开复位内固定术(ORIF)]和观察组(n=26,予以关节镜下MIPPO),观察两组骨折复位情况、愈合时间、膝关节功能及术后并发症发生率。结果:观察组术后1年胫骨平台后倾角(PA)、内翻角(TPA)及股胫角(FTA)角度均较对照组大,骨折愈合时间较对照组短(P<0.05);术后1年,观察组美国特种外科医院(HSS)评分较对照组高,膝关节活动度较对照组大(P<0.05);观察组术后并发症发生率(3.85%)较对照组(23.08%)低(P<0.05)。结论:关节镜下MIPPO治疗胫骨平台骨折可促进术后骨折复位、缩短骨折愈合时间、改善术后膝关节功能,并且降低术后并发症发生率。Objective:To investigate the value of minimally invasive percutaneous plate osteosynthesis(MIPPO)under arthroscopy in the treatment of tibial plateau fractures.Method:Fifty-two cases of tibial plateau fracture treated in our hospital from January 2015 to March 2019 were selected as the study objects,and divided into the control group[n=26,treated with open reduction and internal fixation(ORIF)]and the observation group(n=26,treated with MIPPO)according to simple randomization method.Fracture reduction,fracture healing time,knee joint function and incidence of postoperative complications of the two groups were observed.Result:The posterior slope angle(PA)of tibial plateau,varus angle of tibial plateau(TPA)and femor-tibial angle(FTA)in the observation group were larger than those in the control group,and the fracture healing time in the observation group was earlier than that in the control group(P<0.05).One year after surgery,the hospital for special surgery(HSS)score of the observation group was higher than that of the control group,and the range of motion of the knee joint in the observation group was higher than that in the control group(P<0.05).The incidence of postoperative complications in the observation group was 3.85%,which was lower than 23.08%in the control group,there was statistical significance(P<0.05).Conclusion:MIPPO under arthroscopy treatment of tibial plateau fracture can promote postoperative fracture reduction,shorten fracture healing time,improve knee joint function and reduce the incidence of postoperative complications.
