彩色多普勒超声与D-二聚体诊断下肢深静脉血栓的价值

摘　　要：目的:分析彩色多普勒超声与D-二聚体在下肢深静脉血栓患者诊断过程中的临床应用价值。方法:将2018年3月~2019年3月本院收治的疑似发生下肢深静脉血栓的患者54例作为观察组,选取同期在本院接受健康体检的正常人54例作为对照组,均为其提供彩色多普勒超声检查和D-二聚体检查,分析单独使用两种诊断方法和联合运用两种诊断方法过程中的效果与价值。结果:联合诊断敏感度、特异性、阳性预测值和阴性预测值与单独运用彩色多普勒检查或者是D-二聚体检查存在显著差异(P<0.05);观察组患者急性期和亚急性期D-二聚体水平明显高于对照组正常体检者(P<0.05);观察组患者慢性期D-二聚体水平与对照组体检者相比较高,但是差异无统计学意义(P>0.05)。结论:彩色多普勒超声与D-二聚体具有着一定的效果,其中D-二聚体水平可能受发病时期的影响,但是其具有着相对较好的预测作用,因此,在实际诊断下肢深静脉血栓的过程中可以联合运用彩色多普勒超声与D-二聚体。Objective:To analyze the clinical value of color Doppler ultrasound and D-dimer in the diagnosis of patients with lower extremity thrombosis.Methods:54 patients with suspected lower limb thrombosis in our hospital from March 2018 to March 2019 were selected as the observation group,and 54 normal people who received physical examination in our hospital were selected as the control group,color Doppler ultrasonography and D-dimer examination were provided to analyze the effect and value of the two diagnostic methods alone and in combination.Results:There were significant differences between the combined diagnostic sensitivity,specificity,positive and negative predictive values and the color Doppler examination or D-dimer examination alone(P<0.05)The level of D-dimer in the observation group was higher than that in the control group(P<0.05),and the level of D-dimer in the observation group was higher than that in the control group,but the difference was not statistically significant(P>0.05).Conclusion:Color Doppler ultrasound and D-dimer have certain effect,in which D-dimer level may be affected by the time of onset,but it has a relatively good predictive role,therefore,a combination of color Doppler ultrasound and D-dimer can be used in the actual diagnosis of thrombosis in the lower extremities.
