135例着丝点抗体阳性患者的临床回顾性研究

摘　　要：目的分析135例抗核抗体中着丝点抗体阳性的免疫性疾病分布及其潜在临床意义。方法获取135例自身免疫性疾病患者外周血后离心,并应用德国欧盟诊断试剂及间接免疫荧光方法检测抗核抗体滴度,联合免疫印迹半定量方法检测B亚型着丝点抗体,分析着丝点抗体阳性的自身免疫性疾病的分布和潜在临床意义。结果135例着丝点抗体阳性患者中,1:32滴度抗核抗体14例,1:100滴度26例,1:320滴度56例,1:1000滴度39例。135例患者中包括35例干燥综合征患者,26例类风湿性关节炎和关节痛患者,19例系统性红斑狼疮患者,17例结缔组织病患者,14例免疫性不孕和流产患者,12例硬皮病患者,6例雷诺现象患者,2例皮肌炎患者,湿疹、甲亢和骨关节炎患者各1例。结论免疫性疾病中合并着丝点抗体阳性率为最高的疾病干燥综合征、类风湿性关节炎和系统性红斑狼疮患者。Objective To analyze the distribution and potential clinical significance of positive centromere antibodies in 135 autoimmune disease cases of anti-nuclear antibodies.Methods Antinuclear antibodies were detected by indirect immunofluorescence assay and subtype B centromere antibodies were detected by semi-quantitative western blot method in 135 patients with autoimmune diseases.Then the distribution and potential clinical significance of 135 positive centromere antibodies autoimmune diseases were analyzed.Results Among the positive centromere antibody,the antinuclear antibodies included 14 cases(1:32 titers),26 cases(1:100 titers),56 cases(1:320 titers)and 39 cases(1:1000 titers).Among them,35 cases were Sjogren's syndrome,26 cases were rheumatoid arthritis,19 cases were systemic lupus erythematosus,17 cases were connective tissue disease,14 cases were immunological infertility,12 cases were scleroderma,6 cases were Reynolds phenomenon,2 cases were dermatomyositis,1 case was eczema,1 case was hyperthyroidism and 1 case was osteoarthritis.Conclusion The positive rate of centromere antibodies were higher in primary Sjogren's syndrome,rheumatoid arthritis and systemic lupus erythematosus patients.
