穿支蒂螺旋桨皮瓣治疗小腿皮肤软组织缺损40例

摘　　要：目的:研究小腿皮肤软组织缺损患者应用穿支蒂螺旋桨皮瓣治疗的疗效及预后影响因素。方法:回顾性分析2016年3月到2019年3月治疗的40例行穿支蒂螺旋桨皮瓣治疗的小腿皮肤软组织缺损患者的临床资料,观察患者术后疗效及并发症发生情况,并根据并发症有无情况分为有并发症组和无并发症组,分析影响并发症的相关因素。结果:40例患者术后总优良率为95.0%,并发症发生率为37.5%;有并发症患者和无并发症患者在性别、年龄以及BMI上差异无统计学意义(P>0.05),但在皮瓣长度、蒂部长度、旋转角度上无并发症组均高于有并发症组,差异有统计学意义(P<0.05)。Logistic回归结果显示,皮瓣长度、蒂部长度、旋转角度虽在一定程度上影响患者术后并发症的发生,但均非导致患者发生并发症的独立影响因素,差异无统计学意义(P>0.05)。结论:治疗小腿皮肤软组织缺损时,穿支蒂螺旋桨皮瓣的总体疗效良好,但需要在皮瓣长度、蒂部长度、旋转角度上不断调整,减少后期并发症的发生。Objective:To study the curative effect and prognostic factors of patients with skin and soft tissue defect of leg treated with pedicled propeller flap.Methods:The clinical data of 40 patients with skin and soft tissue defects of the lower leg treated with perforating pedicled propeller flap during the period from March 2016 to March 2019 were retrospectively analyzed,to observe the postoperative curative effect and the occurrence of complications,and the patients were divided into the complication group and the non-complicated group,and to analyze the related factors affecting the complications.Results:The overall excellent and good rate of 40 patients was 95.0%,and the complication rate was 37.5%.There was no significant difference in sex,age and BMI between patients with and without complications(P>0.05).However,the flap length,pedicle length and rotation angle in the non-complication group were higher than those in the complication group(P<0.05).The results of Logistic regression showed that the length of flap,the length of pedicle and the angle of rotation affect the occurrence of postoperative complications to some extent,but they were not independent factors leading to complications(P>0.05).Conclusion:In the treatment of skin and soft tissue defect in the lower leg,the overall effect of the pedicle propeller flap is good,but it needs to continuously adjust the length of the flap,the length of the pedicle,and the rotation angle to reduce the occurrence of late complications.
