肺保护性通气治疗外科患者术后吸入性肺炎的疗效

摘　　要：目的探讨肺保护性通气治疗外科患者术后吸入性肺炎的疗效。方法将2016年1月至2017年12月我科收治的32例外科术后吸入性肺炎患者作为对照组,采用常规呼吸衰竭的治疗方法(未实施肺保护性通气治疗);将2018年1月至2019年12月我科收治的33例外科术后吸入性肺炎患者作为试验组,采用肺保护性通气进行治疗。两组患者均发生了严重的ARDS(急性呼吸窘迫综合征),观察两组患者的氧合指数改善情况、抗生素应用时间、气管插管及呼吸机的使用时间、ICU入住时间。结果试验组患者的氧合指数短期内改善效果较明显、ARDS纠正效果理想,抗生素应用时间缩短、气管插管及呼吸机应用时间缩短、ICU入住时间缩短(P<0.05)。结论肺保护性通气治疗有助于外科术后吸入性肺炎患者的恢复。Objective To investigate the eff ect of lung protective ventilation on postoperative aspiration pneumonia in surgical patients.Methods Thirtytwo patients with postoperative aspiration pneumonia admitted to our department from January 2016 to December 2017 were used as the control group,and they were treated with conventional respiratory failure treatment(no lung protective ventilation treatment).Thirty-three patients with postoperative aspiration pneumonia admitted to our department from January 2018 to December 2019 were selected as the experimental group and treated with lung protective ventilation.Severe ARDS(acute respiratory distress syndrome)occurred in both groups of patients.The improvement of oxygenation index,antibiotic application time,tracheal intubation and ventilator use time,and ICU stay time were observed.Results The oxygenation index of the patients in the experimental group improved signifi cantly in the short term,the ARDS correction eff ect was ideal,the antibiotic application time was shortened,the tracheal intubation and ventilator application time was shortened,and the ICU stay time was shortened(P<0.05).Conclusion Lung protective ventilation therapy is helpful for the recovery of patients with aspiration pneumonia after surgery.
