利福喷丁和利福平治疗初治涂阳肺结核患者的临床疗效及不良反应

摘　　要：目的分析比较利福喷丁和利福平治疗初治涂阳肺结核患者的临床疗效及不良反应。方法随机盲选我院收治的初治涂阳肺结核患者82例,将单数患者作为对照组,双数患者作为试验组,对照组41例实施利福平治疗,试验组41例实施利福喷丁治疗。对两组患者的空洞闭合情况、病灶吸收情况、各时间段痰转阴情况以及不良反应发生情况进行比较分析。结果试验组空洞闭合总有效率、病灶吸收率高于对照组(P<0.05)。试验组治疗第2个月末、第4个月末及第6个月末痰转阴率明显高于对照组(P<0.05)。试验组白细胞减少、胃肠道反应、皮疹及肝功能异常发生率低于对照组(P<0.05)。结论和实施利福平治疗相比,对初治涂阳肺结核患者实施利福喷丁治疗具有更为理想的临床疗效,不仅能有效改善患者的空洞闭合情况、病灶吸收情况、各时间段痰转阴情况,还能明显减少不良反应的发生。Objective To analyze and compare the clinical effi cacy and adverse reactions of rifapentine and rifampicin in the treatment of patients with newly treated smear-positive pulmonary tuberculosis.Methods Eighty-two patients with newly treated smear-positive pulmonary tuberculosis in our hospital were randomly and blindly selected.The odd number of patients was taken as the control group,and the even number of patients as the experimental group.Forty-one cases in the control group were treated with rifampicin,and 41 cases in the experimental group were treated with rifapentine.The cavity closure,focus absorption,sputum negative conversion in each time period and adverse reactions were compared between the two groups.Results The total eff ective rate of cavity closure and the rate of focus absorption in experimental group were higher than that in control group(P<0.05).The rate of sputum negative conversion of the experimental group at the end of the second,fourth and sixth month of treatment was signifi cantly higher than that of the control group(P<0.05).The incidence of leukopenia,gastrointestinal reaction,rash and abnormal liver function in the experimental group was lower than that in the control group(P<0.05).Conclusion Compared with the implementation of rifampicin treatment,rifapentine treatment for patients with newly treated smear-positive pulmonary tuberculosis has more ideal clinical eff ect,which can not only eff ectively improve the cavity closure,focus absorption,sputum negative conversion in each time period,but also signifi cantly reduce the occurrence of adverse reactions.
