胎膜早破合并胎盘早剥的相关因素及护理方法

摘　　要：目的探讨影响胎膜早破合并胎盘早剥的因素及有效护理方法。方法此次100例研究对象全是本院2017年1月至2019年1月收治的胎膜早破合并胎盘早剥产妇,回顾分析临床资料总结影响因素,并采用双模拟法分组,给予研究组综合护理干预,常规组接受基础护理干预,对比两组患者的护理效果。结果相关影响因素:32.00%是妊娠期高血压、18.00%是子宫畸形、15.00%是臀位、13.00%是意外性外伤、12.00%是胎儿宫内发育受限、10.00%是其他原因;经过护理,研究组和常规组产妇产褥感染率为8.00%和24.00%,新生儿窒息率为4.00%和18.00%,新生儿感染率为10.00%和26.00%,另外研究组比常规组产后2 h出血量少,差异均呈统计学意义(P<0.05)。结论胎膜早破是造成胎盘早剥的主要因素,同时两种疾病合并发生还与妊娠期高血压、子宫畸形、臀位、意外性伤害等因素相关,一旦确诊要及时进行综合性护理,以优化母婴结局。Objective To explore the factors affecting premature rupture of membranes with placental abruption and effective nursing methods.Methods The 100 cases of pregnant women with premature rupture of membranes and placental abruption admitted to our hospital from January 2017 to January 2019 were retrospectively analyzed.The infl uencing factors were summarized and analyzed.The patients was divided into two groups by double simulation method.The study group was given comprehensive nursing intervention.The routine group received basic nursing intervention,and the nursing eff ect of the two groups was compared.Results Relevant infl uencing factors:32.00%were gestational hypertension,18.00%were uterine malformation,15.00%were buttock position,13.00%were accidental trauma,12.00%were fetal intrauterine growth restriction,10.00%were other causes;after nursing,puerperal infection rate of study group and routine group was 8%and 24.00%,neonatal asphyxia rate was 4.00%and 18.00%,neonatal infection rate was 10%and 26.00%,and the amount of postpartum hemorrhage of study group was 2 lower than that of routine group.The diff erence was statistically signifi cant(P<0.05).Conclusion Premature rupture of membranes is the main cause of placental abruption,at the same time,the combined occurrence of the two diseases is also related to pregnancy hypertension,uterine malformation,buttock position,accidental injury and so on.Once diagnosed,comprehensive nursing should be carried out in time to optimize maternal and infant outcomes.
