关节镜下半月板部分切除术治疗膝关节半月板损伤的远期疗效观察

摘　　要：目的:探讨关节镜下半月板部分切除术治疗膝关节半月板损伤的远期疗效。方法:抽取2012年1月至2014年12月在郑州人民医院行关节镜下半月板部分切除术治疗的80例膝关节半月板损伤患者,术后随访60个月,比较患者术前及术后30、60个月的美国特种外科医院(HSS)膝关节功能评分、Lysholm评分及远期并发症发生情况。结果:所有手术均成功,术后随访无失访病例。术后30、60个月,患者的疼痛、功能、活动度、稳定性及HSS总分均高于术前水平(P均<0.05),且跛行、支撑、交锁、不稳、疼痛、肿胀、爬楼、下蹲个各维度评分及Lysholm总分均高于术前水平(P均<0.05);但术后30个月与术后60个月,患者的HSS评分、Lysholm膝关节功能各维度评分及总分比较差异未见统计学意义(P>0.05),所有患者均未出现并发症。结论:关节镜下半月板部分切除术治疗膝关节半月板损伤能够有效改善患者膝关节功能,远期疗效显著且稳定,且无并发症发生。Objective:To investigate the long-term efficacy of arthroscopic partial meniscectomy for knee joint meniscus injury.Methods:A total of 80 patients with knee meniscus injury who underwent arthroscopic meniscal partial resection in Zhengzhou People’s Hospital from January 2012 to December 2014 were enrolled,and they were followed up for 60 months.The Hospital for special surgery(HSS)knee function score,Lysholm score and long-term complications were compared before operation and 30,60 months after operation.Results:All the operations were successful,and no one was lost during follow-up.At 30 and 60 months after operation,the pain,function,activity,stability and total score of HSS were higher than those before operation(all P<0.05);and the patient’s lameness,support,interlocking,instability,pain,swelling,climbing,squatting,and Lysholm total scores were significantly higher than those before operation(all P<0.05).However,there was no significant difference in HSS score,Lysholm knee function score and total score at 30 months and 60 months after operation(P>0.05).No complication occurred in ang patients.Conclusions:Arthroscopic partial meniscectomy for knee joint meniscus injury can effectively improve knee joint function,of which the long-term effect was significant and stable,without complications.
