摘 要:目的探讨地佐辛超前镇痛对腹腔镜手术患者苏醒期血流动力学的影响。方法抽取2018年5月至2020年5月巴彦淖尔市医院收治的腹腔镜手术患者87例,按照随机数字表简单随机分为对照组(43例)与研究组(44例)。两组患者均在入室后进行静脉输液,使用监护仪密切检测心率(HR),平均动脉压(MAP),血氧饱和度(SpO2),脑电双频指数(BIS)等指标。研究组在手术结束前30 min应用静脉注射地佐辛5 mg,对照组采用同速度注射静脉注射等量生理氯化钠溶液,观察两组患者在手术结束时、苏醒时、拔管时、拔管后5 min、拔管后15 min的血流动力学指标的差异,并在手术结束后比较两组的不良反应发生率。结果重复测量方差分析显示,研究组各时间点HR、MAP、SpO2、BIS的波动幅度都小于对照组,研究组苏醒时、拔管时、拔管后5 min的HR、MAP数值都低于对照组,拔管时的BIS低于对照组,手术结束时、苏醒时的SpO2高于对照组(P<0.05)。研究组患者手术后不良反应发生率少于对照组(P<0.05)。结论地佐辛超前镇痛可稳定腹腔镜手术患者苏醒期血流动力学,降低不良反应发生率。Objective To explore the effect of dezocine preemptive analgesia on the hemodynamics of patients undergoing laparoscopic surgery during awakening.Methods A total of 87 cases of laparoscopic surgery patients admitted to Bayannur Hospital from May 2018 to May 2020 were selected.According to the random number table,the patients were randomly divided into control group(43 cases)and study group(44 cases).Patients in both groups received intravenous infusion after entering the room,and heart rate(HR),mean arterial pressure(MAP),blood oxygen saturation(SpO2),bispectral index(BIS)and other indicators were closely detected by a monitor.The study group was given dezocine 5 mg intravenously 30 minutes before the end of the operation,while the control group was injected with the same volume of physiological sodium chloride solution at the same speed.The hemodynamic indexes of the two groups were observed at the end of operation,recovery,extubation,5 min and 15 min after extubation,the incidence of adverse reactions was compared between the two groups after operation.Results Repeated measurement analysis of variance showed that the fluctuation ranges of HR,MAP,SpO2 and BIS in the study group were smaller than those in the control group at each time point,the HR and MAP values of the study group were lower than those of the control group at awakening,extubation and 5 min after extubation,BIS of extubation group was lower than that of control group,SpO2 at the end of operation and recovery was higher than that of control group(P<0.05).The incidence of adverse reactions in the study group was less than that in the control group(P<0.05).Conclusions Dezocine preanalgesia can stabilize hemodynamics and reduce the incidence of adverse reactions in patients undergoing laparoscopic surgery.
