皮下组织蒂菱形皮瓣成形术治疗烧伤后瘢痕挛缩的临床效果

摘　　要：目的探讨皮下组织蒂菱形皮瓣成形术治疗烧伤后瘢痕挛缩的临床效果。方法抽取2017年8月至2019年2月晋城大医院收治的烧伤后瘢痕挛缩患者75例,依据手术方式将其分为两组,观察组38例行皮下组织蒂菱形皮瓣成形术,对照组37例行常规游离皮片移植术。比较两组患者皮瓣成活率、瘢痕挛缩复发率及生活质量评分。结果观察组术后皮瓣成活率(100.00%,38/38)高于对照组(91.89%,34/37),但差异未见统计学意义(P>0.05)。随访1年,观察组瘢痕挛缩复发率(5.26%,2/38)低于对照组(21.62%,8/37),差异有统计学意义(P<0.05)。术后6、12个月,观察组生活质量评分高于对照组(P<0.05)。结论皮下组织蒂菱形皮瓣成形术皮瓣成活率高,远期瘢痕挛缩复发率低,可改善患者生活质量。Objective To investigate the clinical effect of subcutaneous tissue pedicled rhomboid flap on scar contracture after burn.Methods A total of 75 patients with scar contracture after burn admitted to Jincheng Coal Industry Group General Hospital from August 2017 to February 2019 were selected and divided into two groups according to the surgical protocols.Thirty-eight patients in the observation group treated with subcutaneous tissue pedicle rhomboid skin flaps,and 37 patients in the control group treated with conventional free skin grafts.The flap survival rate,scar contracture recurrence rate and quality of life scores were compared between the two groups.Results The postoperative flap survival rate in the observation group(100.00%,38/38)was higher than that in the control group(91.89%,34/37),but the difference was not significant(P>0.05).After 1-year follow-up,the recurrence rate of scar contracture in the observation group(5.26%,2/38)was lower than that in the control group(21.62%,8/37),and the difference was significant(P<0.05).At 6 and 12 months after operation,the quality of life score of the observation group was higher than that of the control group(P<0.05).Conclusions Subcutaneous tissue pedicle rhomboid flaps have a high survival rate and low recurrence rate of long-term scar contracture,which can improve the quality of life of patients.
