放散式冲击波联合关节松动术治疗非巨大肩袖损伤的效果分析

摘　　要：目的:探讨放散式冲击波联合关节松动术治疗非巨大肩袖损伤的疗效。方法:选取笔者所在医院门诊2019年1月-2020年2月就诊的非巨大肩袖损伤患者57例,将其随机分为冲击波组29例和对照组28例。对照组患者只接受关节松动术治疗,冲击波组在对照组治疗基础上加用放散式冲击波治疗,观察两组治疗前后的肩关节疼痛视觉模拟量表(VAS评分)、美国加州大学肩关节评分(UCLA)及肩关节活动度(ROM)。结果:本次临床试验后,治疗后两组患者的VAS评分、UCLA得分及肩关节活动度(ROM)均有显著改善,然而冲击波组的VAS评分显著低于对照组[(2.78±0.35)分vs(4.26±0.86)分,P<0.01]、UCLA得分显著高于对照组[(33.72±1.56)分vs(26.37±2.47)分,P<0.01],肩关节活动度显著优于对照组(P<0.01)。结论:放散式冲击波联合动态关节松动术可明显减轻肩部疼痛,改善肩关节功能,可作为非巨大肩袖损伤的常规治疗方法,值得临床应用。Objective:To evaluate the effect of diffuse shock wave combined with joint mobilization on no serious rotator cuff injury.Method:From January 2019 to February 2020,57 patients with no serious rotator cuff injury were randomly divided into shockwave group(29 cases)and control group(28 cases)in the outpatient department of rehabilitation of our hospital.Outcomes will be measured before and after this clinical trial,including the visual analog scale(VAS),UCLA score and shoulder range of motion.Result:After clinical trial,the visual analog scale(VAS),UCLA score and shoulder range of motion of two groups were significantly better than before(P<0.05).After clinical trial,the visual analog scale(VAS)were significantly lower in the shockwave group than in the control group[(2.78±0.35)points vs(4.26±0.86)points,P<0.01].UCLA score were significantly higher in the shockwave group than in the control group[(33.72±1.56)points vs(26.37±2.47)points,P<0.01].Shoulder range of motion were significantly better in the shockwave group than in the control group(P<0.05).Conclusion:Shock wave combined with joint mobilization can significantly release the pain and improve the function of affected shoulder and it can be as an regular treatment for rotator cuff injury,and worthy of promotion and application.
