基于循证和Delphi法构建“心脏康复阻力因素问卷”的研究

摘　　要：目的构建“心脏康复阻力因素问卷”,为顺利开展心脏康复提供必要的评估工具。方法基于循证医学工具制定检索策略,检索国内外数据库获取原始文献,采用Johns Hopkins证据等级与质量评价工具评定文献质量等级,初拟“心脏康复阻力因素问卷”条目池,然后进行两轮德尔菲专家咨询,最终构建“心脏康复阻力因素问卷”。结果共21篇文献纳入研究,通过两轮专家咨询构建“心脏康复阻力因素问卷”,由患者、家庭、医务人员、社会与环境组成一级指标4项,二级指标13项,三级指标44项。两轮专家咨询的积极系数均为100%,专家的权威系数为0.85。两轮专家咨询后,各项指标重要性评分的变异系数为0.00~0.24,可操作性评分的变异系数为0.07~0.24,均小于0.25。结论基于循证和德尔菲法构建的“心脏康复阻力因素问卷”具有较好的实用性和科学性,可指导临床护理实践,为心脏康复的顺利开展提供参考。Objective To construct a"Cardiac Rehabilitation Resistance Factors Questionnaire",to provide necessary evaluation tools for the smooth development of cardiac rehabilitation.Methods A search strategy was developed based on evidence-based medicine tools,and original documents were retrieved from domestic and foreign databases.Johns Hopkins evidence grade and quality evaluation tools were used to assess the quality of the literature.The item pool of the"Cardiac Rehabilitation Resistance Factors Questionnaire"was initially drawn up,and then two rounds of Delphi expert consultation were conducted.The"Cardiac Rehabilitation Resistance Factors Questionnaire"was constructed finally.Results A total of 21 papers were included in the study.The"Cardiac Rehabilitation Resistance Factors Questionnaire"was constructed through two rounds of expert consultation.The questionnaire was composed of patients,families,medical staff,society and the environment,consisting of 4 primary indicators,13 secondary indicators,and 44 tertiary indicators.The positive coefficient of the two rounds of expert consultation was 100%,and the expert’s authority coefficient was 0.85.After two rounds of expert consultation,the coefficient of variation of the importance score of each index was 0.00 to 0.24,and the coefficient of variation of the operability score was 0.07 to 0.24,both less than 0.25.Conclusion The"Cardiac Rehabilitation Resistance Factors Questionnaire"based on evidence-based and Delphi method has good practicality and scientificity,which can guide clinical nursing practice and provide reference for the smooth development of cardiac rehabilitation.
