D-D检测联合ISS评分在急诊创伤患者预后预测中的价值分析

摘　　要：目的探讨D-二聚体(D-D)检测联合创伤严重度评分(ISS)在急诊创伤患者预后预测中的价值。方法回顾性分析2017年5月~2018年5月于我院就诊的186例急诊创伤患者的临床资料,比较不同预后患者的D-D水平和ISS评分;采用受试者工作特征曲线(ROC)比较D-D、ISS评分及联合检测对创伤患者预后的预测价值。结果186例患者中,生存137例,死亡49例;生存组患者D-D水平及ISS评分明显低于死亡组患者(P<0.05);D-D检测、ISS评分及二者联合诊断创伤患者预后的AUC分别为0.794、0.756、0.879,联合检测的AUC明显高于D-D检测和ISS评分(P<0.05)。结论D-D水平、ISS评分与创伤患者预后关系密切,二者联合检测对创伤患者预后预测具有一定价值。Objective To explore the value of D-dimer(D-D)detection combined with Injury severity score(ISS)in prognosis prediction of emergency trauma patients.Methods The clinical data of 186 emergency trauma patients who were admitted in our hospital from May 2017 to May 2018 were retrospectively analyzed.The D-D levels and ISS scores of patients with different prognosis were compared.The receiver operating characteristic curve(ROC)was used to compare the predictive value of D-D,ISS score and combined detection on the prognosis of trauma patients.Results Among the 186 patients,137 survived and 49 died.The D-D level and ISS score of the survival group were significantly lower than those of the death group(P<0.05).The AUC of D-D detection,ISS score and the combined detection of trauma patients was 0.794,0.756 and 0.879.The AUC of the combined detection was significantly higher than that of the D-D detection and ISS score(P<0.05).Conclusion The D-D level and ISS score are closely related to the prognosis of trauma patients.The combined detection of the two has certain value in predicting the prognosis of trauma patients.
