摘  要:目的探讨细胞免疫指标、体液免疫指标与小儿难治性支原体肺炎病情的关系。方法收集我院2016年11月~2019年11月儿科收治的74例小儿支原体肺炎患儿作为研究对象,根据儿童肺炎支原体肺炎诊治专家共识中诊断意见,分为A组(普通支原体肺炎,n=42)与B组(难治性支原体肺炎,n=42)。对比两组细胞免疫指标(CD3+、CD4+)与体液免疫指标[免疫球蛋白G(IgG)、免疫球蛋白M(IgM)]的水平变化情况,并分析CD3+、CD4+、IgG、IgM的水平,以及与难治性支原体肺炎患儿病情的关系。结果B组患儿的CD3+为(52.42±4.01)%、CD4+(27.97±4.26)%水平均比A组[(60.01±4.32)%、(33.63±4.15)%]低,差异有统计学意义(t=7.721、5.746,P=0.000、0.000);B组患儿的IgG(10.43±1.16)g/L、IgM为(2.10±0.70)g/L水平均比A组[(7.30±1.13)g/L、(1.43±0.78)g/L]高,差异有统计学意义(t=11.670、3.824,P=0.000、0.000);将小儿难治性支原体肺炎中重度难治性支原体肺炎是否发生作为因变量(1=中重度,0=轻度),分别将CD3+、CD4+、IgG、IgM作为协变量,经二元Logistic回归分析检验结果显示CD3+、CD4+、IgG、IgM均可能是中重度难治性支原体肺炎的影响因素(OR>1,P<0.05)。结论细胞免疫、体液免疫与小儿难治性支原体肺炎密切相关,检测其指标对小儿难治性支原体肺炎具有诊断意义,并能够有效观察小儿难治性支原体肺炎的病情严重程度,值得临床推广应用。Objective To investigate the relationship between indexes of cellular immunity and humoral immunity and the condition in children with refractory mycoplasma pneumonia.Methods A total of 74 children with mycoplasmal pneumonia admitted to our hospital from November 2016 to November 2019 in our hospital were selected as the research objects.According to the diagnostic opinions of the diagnosis and treatment of children with mycoplasma pneumonia,they were divided into group A(general mycoplasma pneumonia,n=42)and group B(refractory mycoplasma pneumonia,n=42).The changes in the levels of cellular immune indexes(CD3+,CD4+)and humoral immune indexes[immunoglobulin G(IgG),immunoglobulin M(IgM)]were compared.The relationship betweenCD3+,CD4+,IgG,IgM levels and the condition of children with refractory mycoplasma pneumonia was analyzed.Results The CD3+(52.42±4.01)%and CD4+was(27.97±4.26)%levels of children in group B were lower than those in group A[(60.01±4.32)%,(33.63±4.15)%],and the difference was statistically significant(t=7.721,5.746,P=0.000,0.000).The IgG(10.43±1.16)g/L and IgM(2.10±0.70)g/L levels in group B were higher than those of group A[(7.30±1.13)g/L and(1.43±0.78)g/L],and the differences were statistically significant(t=11.670,3.824,P=0.000,0.000).The incidence of refractory mycoplasma pneumonia in children with moderate or severe refractory mycoplasma pneumonia was used as the dependent variable(1=moderate to severe,0=mild),and CD3+,CD4+,IgG,IgM were used as covariates.The binary logistic regression analysis showed that CD3+,CD4+,IgG,and IgM may all be influencing factors of moderate to severe refractory mycoplasma pneumonia(OR>1,P<0.05).Conclusion Cellular immunity and humoral immunity are closely related to refractory mycoplasmal pneumonia in children.The detection of their indicators has diagnostic significance for refractory mycoplasmal pneumonia in children and can help to observe the severity of refractory mycoplasmal pneumonia in children effectively,which is worthy of clinical application.
