摘 要:目的:探究CT三维重建技术用于指导鹿角形肾结石经皮肾镜取石术穿刺定位的作用。方法:选取2016年2月-2020年1月本院放射科收治的120例鹿角形肾结石患者为研究对象,并根据随机数字表法将研究对象平分对照组(60例)与观察组(60例),所有患者均进行经皮肾镜取石术,对照组采用静脉肾盂造影(IVP)指导,而观察组采用CT三维重建技术指导,比较两组患者的术中出血量、穿刺通道数目、手术时间以及结石清除率。结果:对照组与观察组的术中出血量相当,差异无统计学意义(P>0.05);观察组的穿刺通道数目少于对照组,手术时间短于对照组;同时观察组的结石清除率(93.33%)高于对照组(80.00%),差异具有统计学意义(P<0.05)。结论:CT三维重建技术用于指导鹿角形肾结石经皮肾镜取石术穿刺定位,能有效减少穿刺通道数目,缩短手术时间,且结石清除率高。Objective:to explore the role of CT three-dimensional reconstruction in guiding the puncture location of staghorn kidney stones by percutaneous nephrolithotomy.Methods:selection in February 2016-January 2020 in our hospital radiology department of 120 patients with staghorn shape kidney stones as the research object, and according to random number table method to divide the research object in the control group(60 cases) and observation group(60 cases), all patients were performed percutaneous nephrolithotomy, and the control group by intravenous urography angiography(IVP) guidance, the observation group with CT 3 d reconstruction technique guidance, compared two groups of patients with intraoperative blood loss, puncture channel number, operation time and stone clearance rate.Results:the amount of intraoperative blood loss was similar between the control group and the observation group, with no significant difference(P>0.05). The number of puncture channels in the observation group was less than that in the control group, and the operation time was shorter than that in the control group. At the same time, the stone clearance rate of the observation group(93.33%) was higher than that of the control group(80.00%), and the difference was statistically significant(P<0.05).Conclusion:CT three-dimensional reconstruction technique can effectively reduce the number of puncture channels, shorten the operation time and improve the stone clearance rate.
