摘 要:目的观察慢性肾衰竭(CRF)合并冠心病患者的血清胱抑素C(Cys C)表达状况,分析Cys C水平与CRF合并冠心病患者微炎症状态的相关性。方法回顾性分析2018年1月至2019年6月于本院接受治疗的CRF患者的临床资料,从中筛选出合并冠心病的50例患者纳入冠心病组,筛选出未合并冠心病的50例患者纳入无冠心病组,另选择同期本院内体检证实健康的50例体检者纳入对照组。比较两组CRF患者的血肌酐(Scr)、24 h尿蛋白定量(24 h-Upr)水平;对比分析三组研究对象的Cys C、高敏C反应蛋白(hs-CRP)、白介素6(IL-6)、肿瘤坏死因子α(TNF-α)水平,采用双变量Pearson直线相关性分析CRF合并冠心病患者血清Cys C水平与各炎症因子水平的相关性。结果冠心病组的血清Cys C、hs-CRP、IL-6、TNF-α水平最高,后由高至低依次为无冠心病组、对照组,组间比较差异有统计学意义(P<0.05);双变量Pearson直线相关性分析结果显示,CRF合并冠心病患者的血清Cys C水平与hs-CRP、IL-6、TNF-α水平均呈正相关(r>0,P<0.05)。结论 CRF合并冠心病患者普遍存在微炎症状态,这种微炎症状态与患者血清Cys C水平表达有关;Cys C可能参与了CRF合并冠心病患者微炎症状态的发生;这可为CRF合并冠心病患者早期微炎症状态的评估与治疗提供新思路。Objective To observe the expesion of serum cytatin C(Cys C)in patients with chronic renal failure(CRF)and coronary heart disase,and to analyze the rlationship between Cys c level and micno-inflammatory state in patients with CRF and coronary heart disease.Methods The clinical data of patients with CRF who treated in the hospital from January 2018 to June 2019 were ret-rospectively analyzed.Fify cases with coronary heart disease were ineluded in coronary heart disease group,50 cases without coronary heart disease were included in non coronary heart disease group,and 50 healthly controls who were confirmned by physical examination during the same period were ineluded in control goup.The serum creatinine(Sr)and 24 hour urinary protein(24 h Upr)levels of CRF pa-tients in the two groups were compared.The Cys C,high sesitivity C-reactive protein(hs-CRP),inter-leukin 6(IL-6),and tumor necrosis factor a(TNF-a)levels of the three goups were compared.The crrelation between serum Cys-C levels and the levels of inflanmatory factors in paients with CRF and coronary heat disease was analyzed by bivariate Pearson linear errelation analysis.Results The lev-els of serum Cys Chs-CRP,IL6 and TNF-a in coronary heart disease group was the hghest,fllowed by non-coronary heart discase group and control group,and the dfference between groups was signif-canl(P<0.05).Bivariate Pearson linear corelation analysis showed that the serum Cys c level was positively corelated with hs-CRP,I6 and TNF-a in patients with CRF and coronary heart disease(r>0,P<0.05).Conclusions Micro-inflammalory state is common in patients with CRF and coro-nary heart disease.This miero-inlammatory state is related io the expression of serum Cys C in pa-tients.Cys C may be involved in the ocurrence of miero-ifanmatory state in patients with CRF and coronary heart disease,which may provide new ides for the asessment and treatment of early micro-in-flammatory state in patients with CRF and coronary heart disease.
