血压变异性与缺血性脑卒中患者脑白质病变严重程度的关系及WML的影响因素分析

摘　　要：目的分析血压变异性与缺血性脑卒中(CIS)患者脑白质病变(WML)严重程度的关系及WML的影响因素。方法将辽宁省人民医院2017年12月-2019年7月间收治70例CIS患者依照患者MRI诊断结果分为WML组及非WML组,WML组分为轻度组、重度组;采用线性模型分析血压变异性与WML严重程度的关系,logistics回归模型分析各种因素与WML的关系。结果WML组患者24h舒张压变异性(24hDBPV)、日昼收缩压变异性(dSBPV)及夜间收缩压变异性(nDBPV)水平均明显高于非WML组,且差异有统计学意义(P<0.05);轻度组患者24hDBPV、dSBPV及nDBPV水平均明显低于重度组,且差异有统计学意义(P<0.05);CIS患者24hDBPV、dSBPV及nDBPV水平与WML呈显著正相关关系(P<0.05);WML组患者年龄明显高于非WML组,且差异存统计学意义(P<0.05);dSBPV及年龄是影响CIS患者WML的独立性影响因素(P<0.05)。结论CIS患者24hDBPV、dSBPV及nDBPV水平与WML呈显著正相关关系,且dSBPV及年龄是影响CIS患者WML的独立性影响因素。Objective To analyze the relationship between blood pressure variability and the severity of WML in CIS patients.Methods Seventy patients with CIS treated in our hospital from December 2017 to July 2019 were divided into WML group and non-WML group according to the MRI diagnosis results of patients,WML components were mild group and severe group;linear model was used to analyze blood pressure variability The relationship with the severity of WML,the logistic regression model analyzes the relationship between various factors and WML.Results The levels of 24hDBPV,dSBPV,and nDBPV in patients in the WML group were significantly higher than those in the non-WML group,and the differences were statistically significant(P<0.05).The levels of 24hDBPV,dSBPV,and nDBPV in patients in the mild group were significantly lower than those in the severe group,and the differences were statistically significant.Significance(P<0.05);24hDBPV,dSBPV,and nDBPV levels in patients with CIS showed a significant positive correlation with WML(P<0.05);patients in the WML group were significantly older than the non-WML group,and the differences were statistically significant(P<0.05);DSBPV and age were independent influencing factors of WML in CIS patients(P<0.05).Conclusion The levels of 24 h DBPV,dSBPV and nDBPV in CIS patients were significantly positively correlated with WML,and dSBPV and age were independent factors that affected WML in CIS patients.
