甘肃省新报告的117例16~25岁人类免疫缺陷病毒1型感染者pol区基因分型及耐药分析

摘　　要：目的了解甘肃省117例新报告的16~25岁人类免疫缺陷病毒1型(human immunodeficiency virus-1,HIV-1)感染者基因型特征和耐药毒株的流行情况。方法采集2015-2016年和2018年甘肃省117例新报告的16~25岁未接受抗病毒治疗的HIV-1感染者的血浆样本。提取并扩增HIV-1 pol区基因序列,进行测序和耐药基因型分析。结果3年共获得HIV-1 pol区基因序列103条,主要以CRF07_BC为主(55.34%,57/103),其次为CRF01_AE(29.13%,30/103),发现了少量CRF08_BC(6.80%,7/103)、B亚型(1.94%,2/103)和CRF55_01B(6.80%,7/103)。4例患者(3.88%,4/103)出现耐药突变,突变位点分别针对蛋白酶类抑制剂(protease inhibitors,PIs)和非核苷类抑制剂(non-nucleoside reverse transcriptase inhibitors,NNRTIs),各为2例。结论甘肃省117例16~25岁新报告的HIV-1感染者中主要HIV-1流行毒株为CRF07_BC,首次发现CRF55_01B重组亚型,应引起重视。甘肃省HIV-1新报告感染者耐药传播处于低度传播水平,但有上升趋势,应持续加强耐药监测。Objective To investigate the genotypic characteristics and the prevalence of drug-resistant strains of 117newly reported human immunodeficiency virus infected patients in Gansu province of China.Methods Plasma samples were collected from 117newly reported HIV infected patients aged 16-25years who were not receiving antiviral treatment in 2015,2016and 2018in Gansu province of China.The amplified HIV-1 pol gene region was extracted and sequenced.Drug resistance genotype analyses were carried out from these samples.Results Totally 103HV-1 pol sequences were obtained in three years,mainly CRF07_BC(55.34%,57/103),followed by CRF01_AE(29.13%,30/103),a small number of CRF08_BC(6.80%,7/103),B subtypes(1.94%,2/103)and CRF55_01B(6.80%,7/103)were found.In these samples,drug resistant mutations were found in four cases(3.88%),two cases of them were resistant to protease inhibitors(PIs),two others were resistant to non-nucleoside reverse transcriptase inhibitors(NNRTIs).Conclusions Among the 117newly reported HIV infected persons aged 16-25in Gansu province of China,the main epidemic HIV strain is CRF07_BC.CRF55_01Brecombinant subtype is found for the first time in Gansu province.The prevalence of drug resistant strains in newly reported HIV-1cases in Gansu province is at a low level,but it is on the rise.
