摘 要:目的对河北省儿童医院分离的410株肺炎链球菌进行分型研究,分析其菌型特点,为控制本地区儿童肺炎链球菌感染和疫苗的正确选择提供依据。方法将菌株进行核酸提取,采用普通PCR方法进行种属鉴定,多重PCR方法进行血清学分型。结果 410株肺炎链球菌(Streptococcus pneumoniae,Spn)菌株大部分分离自2岁以下儿童,占82.68%,2~3岁患儿占11.22%,4~5岁患儿占3.9%,>5岁的患儿仅占2.2%。年龄最大患儿为10岁,最小为37 d。410株Spn菌株中菌型占比前3位的是19F、6A/6B和19A,7价、13价和23价肺炎链球菌疫苗的菌型覆盖率分别是55.61%、71.46%、76.83%。结论本地区儿童肺炎链球菌菌型型别丰富,从疫苗菌株覆盖率和保护性来看,提示13价多糖结合疫苗(13-valent pneumococcal conjugate vaccine, PCV13)对本地区儿童可产生较好的预防效果。Objective To provide the basis for controlling of Streptococcus pneumoniae’s infection and the correct choice of vaccine in this area,410 strains of Streptococcus pneumoniae isolated from children in Hebei children’s Hospital were classified and analyzed the serotype characteristics of Streptococcus pneumoniae infected in children.Methods DNA was extracted from 410 strains of Streptococcus pneumoniae.The species were identified by common PCR and serotyped by multiplex PCR.Results Most of the 410 Spn strains were isolated from children under 2 years old,accounting for 82.68%,2-3 years old accounting for 11.22%,3-4 years old accounting for 3.9%and>5 years old accounting for 2.2%.The oldest child was 10 years old and the youngest was 37 days.Among the 410 Spn strains,the top three serotypes were 19 F,6 a/6 B and 19 A.The coverage rates of 7 valent,13 valent and 23 valent Streptococcus pneumoniae vaccines were 55.61%,71.46%,76.83%,respectively.Conclusion There are many serotypes of Streptococcus pneumoniae in children in this area.From the perspective of coverage rate and protection of vaccine strains,it is suggested that 13 valent PCV vaccine can produce better prevention effect on children in this area.
