基于TOPSIS法的陕西省2017-2019年医疗机构传染病报告质量评价

摘　　要：目的了解陕西省医疗机构法定传染病报告管理现状,进一步提高我省传染病报告质量。方法 2017-2019年采用分层多阶段随机抽样的方法每年抽取3个市,每个市随机抽取2~4个县/区,每个县/区随机抽取综合医院1家,乡镇卫生院/社区卫生服务中心1~2家进行现场调查,计算法定传染病报告率、报告及时率、纸质传染病报告卡填写完整率和准确率、报告卡与网络报告信息一致率和有效证件号填写率,并利用TOPSIS法进行报告质量综合评价。结果2017-2019年陕西省共抽取65家医疗机构,现场核查乙丙类传染病26种2 492例,其中2 476例进行网络直报,全省医疗机构法定传染病报告率为99.36%,不同级别医疗机构传染病报告率差异具有统计学意义(χ^2趋势2=10.920,P=0.001)。报告及时率、传报卡填写完整率和准确率、报告卡与网络报告信息一致率、有效证件号填写率分别99.96%、93.37%、87.29%、84.04%和99.47%。二级医疗机构传染病报告质量最高,三级医疗机构次之,一级医疗机构报告质量较差。结论 2017-2019年我省传染病报告质量总体较好,但仍存在漏报、报告不及时、报告准确率、一致率较差等问题,应进一步提升传染病报告质量。Objective The aim of this study was to understand current situation of infectious disease report and management in Shaanxi medical institutions,and to improve the report quality of infectious disease in Shaanxi province.Methods We selected 3 cities every year by stratified multi-stage random sampling method.And 2-4 counties/districts were randomly selected in each city.One general hospital and 1-2 township health centers/community health service centers were randomly selected in each county/district for filed investigation from 2017 to 2019.The reporting rate of notifiable communicable disease,the report timeliness rate,the completeness and accuracy of reporting cards,the consistency rate of report card and network report information and the valid ID number filling rate were calculated.TOPSIS method was used for comprehensive evaluation of report quality.Results A total of 65 medical institutions were selected in Shaanxi province from 2017 to 2019.2492 cases of 26 infectious diseases(b and c class)were checked,in which 2476 cases were reported.The reporting rate of notifiable communicable disease was 99.36%,and the difference of reporting rates among different medical institutions was statistically significant(trendχ^2=10.920,P=0.001).The report timeliness rate,the completeness and accuracy of reporting cards,the consistency rate of report card and network report information,the valid ID number filling rate were 99.96%,93.37%,87.29%,84.04%and 99.47%,respectively.The report quality of infectious diseases in secondary medical institutions was the highest,followed by tertiary medical institutions,and was poor in primary medical institutions.Conclusion On the whole,the quality of reporting in Shaanxi province was good,but there were still some problems such as underreporting,delay in reporting,poor accuracy rate and poor consistency rate,so the quality of the reporting should be further improved.
