摘  要:为了考察腹泻相关气单胞菌的分布、毒力基因及耐药性。本研究在2015年3月至2018年12月期间共检测了1753例急性腹泻患者的粪便标本,通过全自动微生物鉴定及药敏分析仪对气单胞菌进行鉴定和药敏实验,并检测了8种气单胞菌毒力基因。结果显示,从1753份粪便标本中共分离出67株气单胞菌,检出率为3.82%。气单胞菌类型共包括9种,其中豚鼠气单胞菌、维氏气单胞菌和嗜水气单胞菌为主要气单胞菌,构成比分别为34.33%、22.39%和16.42%。8种气单胞菌毒力基因中,检出率从高到低依次为gcat(73.13%)、Exu(61.19%)、ahyB(50.75%)、act(43.28%)、eprCAL(40.30%)、alt(26.87%)、hlyA(16.42%)和aexT(11.94%)。气单胞菌对头孢唑林(89.55%)、氨苄西林(86.57%)和阿莫西林/克拉维酸(83.58%)的耐药性最高,对阿米卡星(8.96%)、美罗培南(5.97%)和亚胺培南(4.48%)的耐药性最低。ahyB、Exu和EprCAL基因阳性的气单胞菌对头孢曲松、头孢他啶和左氧氟沙星的耐药性显著高于阴性,而AexT阳性的气单胞菌对头孢曲松、头孢他啶和左氧氟沙星的耐药性显著低于阴性(p<0.05)。In order to investigate the distribution virulence genes and drug resistance of Aeromonas-related bacteria.In this study,a total of 1753 fecal samples of patients with acute diarrhea were detected from March 2015 to December 2018.Aeromonas was carried and tested for drug sensitivity by automatic microbial identification and drug sensitivity analyzer,and eight virulence genes of aeromonas were detected.The results showed that 67 strains of Aeromonas were isolated from 1753 fecal samples,with a detection rate of 3.82%.There were 9 species of Aeromonas,among which Aeromonas guinea,Aeromonas vesci,and Aeromonas hydrophila were the main Aeromonas,with composition ratios of 34.33%,22.39%and 16.42%,respectively.Among the 8 Aeromonas virulence genes,the detection rate were gcat(73.13%),Exu(61.19%),ahyB(50.75%),act(43.28%),eprCAL(40.30%),Alt(26.87%),hly A(16.42%),and aex T(11.94%).Aeromonas had the highest resistance to cefazolin(89.55%),ampicillin(86.57%)and amoxicillin/clavulanic acid(83.58%),and the lowest resistance to amikacin(8.96%),meropenem(5.97%)and imipenem(4.48%).The resistance of ahyB,Exu and EprCAL-positive Aeromonas to ceftriaxone,ceftazidine an d levofloxacin were significantly higher than negative,while the resistance of AexT positive Aeromonas to ceftriaxone,ceftazidime and levofloxacin were significantly lower than negative(p<0.05).
