毛囊黏蛋白病11例临床及组织病理特点分析

摘　　要：目的通过对11例毛囊黏蛋白病患者的临床及组织病理特点进行分析,以对该病有进一步的认识。方法收集本科近10年确诊的11例毛囊黏蛋白病患者,对其临床资料、组织病理特点、治疗及预后情况进行回顾性分析。结果该组病例性别差异无统计学意义,平均发病年龄35岁,病程1周~1年余,皮疹好发于头面部(占81.82%),皮疹表现多样,主要表现为浸润性的红斑、丘疹、结节,可伴有毛发脱落,11例中误诊7例。组织病理上表现为毛囊内及其周围的黏蛋白沉积及毛囊周围炎细胞浸润。本组病例1例伴有HIV感染,预后良好。结论该病主要好发于头面部,皮损表现多样,易误诊,组织病理表现为毛囊及其周围的黏蛋白沉积,阿新蓝染色阳性有助于诊断,需注意恶性型(淋巴瘤相关型)的鉴别。Objective To analyze the clinical and histopathological characteristics of 11 patients with follicular mucinosis in order to further understand this disease.Methods We collected 11 cases of follicular mucinosis patients diagnosed in our department in the past 10 years,and retrospectively analyzed their clinical data,histopathological characteristics,treatment and prognosis.Results There were no obvious gender differences in this group of patients.The average age of onset was 35 years and the course of disease was 1 week to more than 1 year.The rash occurred frequently in the head and face(approximately 81.82%).The rash manifestations were diverse,mainly manifested as infiltrating erythema,papules,and nodules,and may be accompanied by hair loss.Seven of 11 cases were misdiagnosed.Histopathology showed the hair follicle to be enveloped by a lymphocytic infiltrate with abundant mucin deposits in and around hair follicles.One case was accompanied by HIV infection,and the prognosis was relatively good.Conclusion The disease mainly occurs in the head and face,and the lesions are diverse,therefore,the disease is more likely to be misdiagnosed.The histopathological manifestations included mucin deposits in and around hair follicles.As Alcian positive blue staining is helpful for diagnosis,attention should be paid to the identification of malignant type(lymphoma-related type).
