关节镜手术在膝关节痛风性关节炎中的诊断及效果观察

摘　　要：目的:探讨关节镜手术在膝关节痛风性关节炎的诊断价值及临床疗效。方法:选取2016年6月-2019年6月在我院行关节镜手术确诊为膝关节痛风性关节炎的患者共25例,记录患者术前、术后1周、末次随访时膝关节Lysholm评分、疼痛VAS评分改善情况。结果:19例患者术前明确诊断为膝关节痛风性关节炎,6例误诊病例经过关节镜探查术中及术后病理结果确诊,其中误诊为半月板损伤3例,慢性滑膜炎2例,骨性关节炎1例;25例手术患者术后均未发生伤口感染及不愈合等并发症,术后1周、末次随访时膝关节Lysholm评分为(56.75±9.63)分、(74.13±5.62)分显著高于术前(35.76±6.43)分(P<0.05);术后1周、末次随访时膝关节VAS疼痛评分(3.04±0.72)分、(2.57±0.4)分显著低于术前(6.81±1.14)分(P<0.05),手术前、后比较差异有统计学意义。结论:关节镜手术对膝关节痛风性关节炎的诊疗效果显著,有助于明确诊断及缓解病情进一步发展,改善膝关节功能,短期疗效满意。Objective To explore the diagnostic value and clinical efficacy of arthroscopic surgery in knee gouty arthritis.Methods A total of 25 patients who were diagnosed with arthroscope surgery for knee gouty arthritis in our hospital from June 2016 to June 2019 were retrospectively recorded.The improvement of Lysholm score and pain VAS score of knee joints before operation,1 week after operation and the last follow-up were recorded.Results 19 patients were clearly diagnosed with gouty arthritis of the knee before operation,and 6 misdiagnosed cases were confirmed by arthroscopic surgery and postoperative pathological results.Among them,3 were misdiagnosed as meniscus injury and 2 were chronic synovitis.One case of osteoarthritis;none of the 25 surgical patients had complications such as wound infection and nonunion.The Lysholm score of knee joint at 1 week after operation and the last follow-up was(56.75+9.63)and(74.13±5.62)significantly higher than that before operation(35.76±6.43)(P<0.05);The knee VAS pain scores(3.04±0.72)and(2.57±0.4)were significantly lower than the preoperative(6.81±1.14)(P<0.05),the difference between before and after operation was statistically significant.Conclusion Arthroscopic surgery has a significant effect on the diagnosis and treatment of gouty arthritis of the knee.It is helpful to confirm the diagnosis and relieve the further development of the disease,improve the function of the knee joint,and the short-term effect is satisfactory.
