四肢开放性骨折合并感染患者外周血Th1/Th2平衡及相关细胞因子变化研究

摘　　要：目的分析四肢开放性骨折合并感染患者外周血Th1/Th2平衡及相关细胞因子的变化。方法回顾性分析自2016-01—2018-12诊治的134例合并感染的开放性骨折(感染骨折组)、70例未合并感染的开放性骨折(单纯骨折组)与134例健康体检者(健康组)。比较健康组、单纯骨折组、骨折感染组治疗前、骨折感染组治疗后Th1、Th2、Th1/Th2以及IL-2、IFN-γ、IL-4、IL-10水平。结果健康组与骨折感染组治疗后Th1、Th2、Th1/Th2比较差异无统计学意义(P>0.05);单纯感染组与骨折感染组治疗前Th1、Th1/Th2较健康组与骨折感染组治疗后低,Th2水平较健康组与骨折感染组治疗后高,差异有统计学意义(P<0.05)。健康组与骨折感染组治疗后IL-2、IFN-γ、IL-4、IL-10水平比较差异无统计学意义(P>0.05);单纯感染组与骨折感染组治疗前IL-2、IFN-γ、IL-4、IL-10水平较健康组与骨折感染组治疗后低,IL-4、IL-10水平较健康组与骨折感染组治疗后高,差异有统计学意义(P<0.05)。结论四肢开放性骨折合并感染会导致Th1/Th2平衡及IL-2、IFN-γ、IL-4、IL-10水平改变。Objective To analyze the changes of Th1/Th2 balance and related cytokines in patients with open fractures complicated with infection.Methods From January 2016 to December 2018,134 cases of open fractures with infection(fracture of the extremities infection group),70 cases of open fractures without infection(simple fracture group)and 134 healthy persons(healthy group)were retrospectively analyzed.The levels of Th1,Th2,Th1/Th2,IL-2,IFN-γ,IL-4 and IL-10 in healthy group,simple fracture group,and fracture infection group before and after treatment were compared.Results There was no significant difference in Th1,Th2 and Th1/Th2 between the healthy group and the fracture infection group after treatment(P>0.05);the Th1,Th1/Th2 before treatment in the simple infection group and the fracture infection group were lower than those in the healthy group and fracture infection group after treatment,and the Th2 level was higher than that in the healthy group and fracture infection group after treatment,the difference was statistically significant(P<0.05).There was no significant difference in the levels of IL-2,IFN-γ,IL-4 and IL-10 between the healthy group and the fracture infection group after treatment(P>0.05);the levels of IL-2,IFN-γ,IL-4 and IL-10 in the simple infection group and the fracture infection group before treatment were lower than those in the healthy group and the fracture infection group after treatment,and the levels of IL-4 and IL-10 were higher than those in the healthy group and the fracture infection group after treatment(P<0.05).Conclusion Open fractures of the extremities complicated with infection can lead to the changes of Th1/Th2 balance and the levels of IL-2,IFN-γ,IL-4 and IL-10.
