摘  要:目的:探讨血清脂蛋白相关磷脂酶A2(LP-PLA2)与心血管疾病10年风险评分-Framingham危险评分(FRS)间的相关性,以期能更早期发现心血管疾病中的中高危患者。方法:选取2019年1月至2020年1月,连续纳入于我院行造影检查的170例住院患者,收集患者一般资料及生化指标。依据FRS将患者分为低危组、中危组和高危组。分析LP-PLA2水平与FRS的相关性。应用多因素Logistic回归分析FRS的影响因素。结果:纳入170例患者,平均年龄(61.44±9.50)岁,其中男性103例(60.6%)。将FRS评分结果将患者分为低危组96例(56.5%),中危组60例(35.3%),高危组14例(8.2%),随着心血管疾病风险增高, LP-PLA2水平明显升高,低危组(186.2±81.2)μg/L、中危组(222.4±84.5)μg/L、高危组(236.4±77.6)μg/L(P=0.009)。Spearman分析示LP-PLA2与FRS(r=0.1997,95%CI:0.0462~0.3439,P=0.09)成正相关。多因素回归分析显示,校正混杂因素后,LP-PLA2与心血管疾病风险仍具有相关性。结论:LP-PLA2水平升高与心血管风险严重程度有相关性,提示LPPLA2可作为预测心血管中高危风险的重要指标。Objective: Lipoprotein-associated phospholipase A2(LP-PLA2) has been considered as a potential predictive inflammatory cytokines for the prevention of severe stroke and been found to be associated with atherosclerosis and coronary heart disease in recent years. The purpose of this study was to investigate the association between serum LP-PLA2 and the 10-year cardiovascular disease risk score, the Framingham risk score(FRS), for the early detection of middle and high-risk patients with cardiovascular disease. Methods: In this study, a total of patients admitted to our hospital for coronary radiographic examination in 2019-2020 were successively included, and the demographic information and biochemical indicators of the patients were collected. Then patients were divided into low risk group, medium risk group and high risk group according to the Framingham risk score. Kruskal-wallis rank sum test and chi-square test were used to compare the differences of clinical continuous variable data in each group, and chi-square test was used for categorical variables. Spearman correlation was used to analyze the correlation between LP-PLA2 level and Framingham risk score. Multivariate Logistic regression was used to analyze the influencing factors of Framingham risk score. Results: A total of 170 patients(mean age 61.44±9.50) years were included in the study, including 103(60.6%) males. The FRS score was used to group 96 patients(56.5%) in the low-risk group, 60 in the medium-risk group(35.3%), and 14 in the high-risk group(8.2%). With the increased risk of cardiovascular disease, the lp-pla2 level significantly increased,including low-risk group(186.2±81.2) μg/L、medium-risk group(222.4±84.5) μg/L、high-risk group(236.4±77.6) μg/L(P=0.009). Spearman analysis showed a positive correlation between LP-PLA2 and FRS(r=0.1997,95% CI:0.0462-0.3439, P=0.09).Multivariate regression analysis showed that LP-PLA2 was still associated with cardiovascular disease risk after adjusting for confounding factors.
