摘 要:目的对青少年双相障碍(pediatric bipolar disorder,PBD)躁狂相患者的功能磁共振成像信号进行分析,考察大脑双侧对称体素之间的镜像同伦功能连接性(voxel-mirrored homotopic connectivity,VMHC),并研究其与临床认知测试之间的相关性。方法纳入17例PBD躁狂相患者与18例正常青少年(正常对照组)进行研究,进行Stroop色词测试、数字广度测试等认知行为学及心理测试,采集功能磁共振成像信号,计算VMHC,提取患者组呈现两组间VMHC显著差异的脑区的VMHC数值,与临床认知测试进行相关性分析。结果与正常对照组相比,PBD躁狂相患者的VMHC值在颞中回、脑岛、罗兰迪克岛盖、颞上回等脑区显著降低。未发现上述脑区的VMHC数值与认知及心理测试指标之间具有相关性。结论 PBD躁狂相患者发生异常镜像同伦功能连接的脑区累及感觉运动网络和语言网络,为理解PBD躁狂相的脑活动异常机制提供了新的线索。Objective:To investigate the voxel-mirrored homotopic connectivity(VMHC)of resting-state functional magnetic resonance imaging signals in mania patients with pediatric bipolar disorder(PBD)and analyze the correlation between VMHC and clinical tests.Methods:Seventeen PBD-mania patients and eighteen healthy adolescents were enrolled,and cognitive behavioral and psychological tests such as Stroop color word test(SCWT)and digit span test(DST)were carried out.Resting-state functional magnetic resonance imaging signals were collected to calculate the VMHC values.In PBD-mania group,VMHC values of the brain regions showing significant differences between the two groups were extracted to analyze correlation between VMHC and cognitive tests.Results:Compared with the healthy control group,the PBD-manic patients showed significantly decreased VMHC values in the brain regions including middle temporal gyrus(MTG),insula(INS),rolandic(ROL)and superior temporal gyrus(STG).No correlation was found between the VMHC values in these brain regions and the above tests.Conclusion:PBD-manic patients show that abnormal functional connectivity is involved in sensorimotor network and language network,which may provide new clues in understanding PBD.
