集束化护理在创伤性颈髓损伤患者呼吸功能康复中的效果观察

摘　　要：目的探讨集束化护理在创伤性颈髓损伤患者呼吸功能康复中的实施效果。方法选取2017年1月至2018年5月入住的创伤性颈髓损伤患者39例为对照组,在呼吸功能康复中使用常规护理;另取2018年6至2019年4月收治的36例同类病例为研究组,在呼吸功能康复中使用常规加集束化护理。对2组患者呼吸功能康复训练1个月动脉血气分析及肺功能测定结果、呼吸系统并发症发生率进行比较和统计学分析。结果研究组动脉血气分析及肺功能改善均明显优于对照组,肺部感染、肺不张、通气障碍等并发症的发生率明显低于对照组,差异均有统计学意义(P<0.05)。结论集束化护理能有效促进创伤性颈髓损伤患者呼吸功能康复,在预防创伤性颈髓损伤患者肺部感染、肺不张、通气障碍等并发症方面效果明显。Objective To investigate the effects of cluster nursing in the rehabilitation of respiratory function in patients with traumatic cervical spinal cord injury.Methods Thirty-nine patients with traumatic cervical spinal cord injury who were admitted and treated in our hospital from January 2017 to May 2018 were enrolled as control group,who were treated by routine nursing for the rehabilitation of respiratory function,at the same time,the other 36 patients with traumatic cervical spinal cord injury who were admitted and treated in our hospital from June 2018 to April 2019 were enrolled as observation group,on the basis of control group,who were treated by cluster nursing for the rehabilitation of respiratory function.After 1-month respiratory rehabilitation training,the results of arterial blood gas analysis and pulmonary function test as well as the incidence rate of respiratory complications were observed and compared between the two groups.Results The improvement status of arterial blood gas indexes and pulmonary function of patients in observation group was superior to that in control group,and the incidence rates of complications including pulmonary infection,atelectasis and ventilation disturbance in observation group were significantly lower than those in control group(P<0.05).Conclusion Cluster nursing can effectively promote the recovery of respiratory function in patients with traumatic cervical spinal cord injury,which has obvious effects in preventing complications including pulmonary infection,atelectasis and ventilation disturbance in patients with traumatic cervical spinal cord injury.
