摘  要:目的探讨无抽搐电休克治疗(modified electroconvulsive therapy,MECT)对首发精神分裂症的疗效及其对患者睡眠结构的影响。方法采用多导睡眠监测(polysomnographic,PSG)对20例对照组(单用氨磺必利治疗)和30例试验组(氨磺必利联合MECT治疗)在治疗前后进行睡眠监测,观察治疗前后PSG的变化。结果试验组和对照组治疗后PANSS量表得分均下降(P<0.001),但试验组减分率大于对照组(P<0.001)。两组治疗后都能增加总睡眠时间、提高睡眠效率;但试验组更优于对照组(P<0.001),且主要增加在睡眠的N3期,差异具有统计学意义(P<0.001)。结论氨磺必利联合无抽搐电休克治疗较单用氨磺必利治疗效果更好。与单用氨磺必利治疗相比,氨磺必利联合MECT治疗不仅能更大程度地增加总睡眠时间和睡眠效率,而且还能优化睡眠结构。Objective To explore the changes of efficacy and sleep structures before and after modified electroconvulsive therapy(MECT)treatment on first-episode schizophrenia.Methods Polysomnography(PSG)was used to monitor the sleep of 20 patients in the control group(treated with amisulpride alone)and 30 patients in the experimental group(treated with amisulpride combined with MECT)before and after treatment,and the changes of PSG before and after treatment were observed.Results The scores of PANSS in the experimental group and the control group decreased after treatment(P<0.001),however,the score reduction rate of the experimental group was higher than that of the control group(P<0.001).After treatment,the total sleep time and sleep efficiency of both groups were increased,but the experimental group was better than the control group(P<0.001),the difference was statistically significant(P<0.001).Conclusion The therapeutic effect of the combination therapy of amisulpride and MECT is better than that amisulpride single therapy.Compared with the treatment of amisulpride alone,the combination of amisulpride and MECT can not only increase the total sleep time and sleep efficiency,but also optimize the sleep structure.
