摘 要:目的探索B群链球菌感染新生儿的生存状况。方法回顾性分析2016年2月至2019年12月于杭州市第一人民医院接受治疗的146例B群链球菌感染新生儿的临床资料,按照患儿1年预后分为生存组(n=102)和死亡组(n=44)。对比两组临床资料,分析脑膜炎患儿脑脊液化验结果及实验室指标,采用多因素Logistic回归分析可能导致B群链球菌感染患儿死亡的相关因素。结果死亡组平均胎龄显著低于生存组,死亡组早产儿占比显著高于生存组,差别具有统计学意义(t/χ^2值分别为2.214、5.116,均P<0.05),两组患儿性别、出生体重、分娩方式、发病类型差别无统计学意义(均P>0.05);死亡组共发现31例(70.45%)脑膜炎,生存组共发现34例(33.33%)脑膜炎,死亡组蛋白、白细胞计数显著高于生存组(t值分别为2.288、2.547,均P<0.05),两组患儿糖、氯化物差别不具有统计学意义(均P>0.05);死亡组C反应蛋白指标显著高于生存组(t=2.462,P<0.05),血小板、总蛋白、谷丙转氨酶、谷草转氨酶、尿素、肌酐水平两组比较差别无统计学意义(均P>0.05);多因素Logistic回归分析结果显示,胎龄是影响B群链球菌感染后新生儿死亡的独立危险因素,OR值及95%CI为3.012(1.203~7.541),P<0.05。结论胎龄是影响B群链球菌感染患儿生存状况的独立因素,脑脊液蛋白及白细胞水平可作为预测辅助指标。Objective To investigate survival status of newborns infected with group B Streptococcus(GBS).Methods The clinical data of 146 newborns with GBS infection who were treated in Department of Pediatrics,Hangzhou Municipal First People’s Hospital from February 2016 to December 2019 were retrospectively analyzed,and according to the 1-year outcomes,the newborns were divided into survival group(n=102)and death group(n=44).The clinical data of the infants in the two groups were compared,the cerebrospinal fluid(CSF)test results and laboratory indexes of those infants with meningitis were analyzed,and multivariate logistic regression was used to analyze the related factors that may lead to death of the infants with GBS infection.Results The average gestational age of the infants in the death group was significantly lower than that of the infants in the survival group,and the proportion of premature infants in the death group was significantly higher than that in the survival group,there were statistically significant differences in average gestational age and proportion of premature infants between the two groups(t/χ^2=2.214 and 5.116 respectively,both P<0.05).There were no significant differences in gender,birth weight,delivery pattern,and morbidity type between the two groups(all P>0.05).31 cases(70.45%)of meningitis were found in the death group and 34 cases(33.33%)of meningitis were found in the survival group,and in the death group the protein concentration and white blood cell counts(WBC)in CSF of the infants were significantly higher than those of the infants in the survival group(t=2.288 and 2.547 respectively,both P<0.05),while there were no statistically significant differences in sugar and chloride in CSF between the two groups(both P>0.05).C-reactive protein(CRP)level in serum of the infants in the death group was significantly higher than that in the survival group(t=2.462,P<0.05),while in levels of platelets(Plt),total protein(TP),alanine aminotransferase(ALT),aspartate aminotransferase(AST),urea(U)and cr
