摘　　要：目的探讨CT引导下经皮穿刺肺活检在肺周围实性占位病变中的诊断价值。方法回顾性分析2018年2月~2019年12月我院收治的51例周围性肺部占位病变患者临床资料,均进行CT引导下经皮穿刺肺活检对肺周围实性占位病变的检查,记录患者的穿刺成功率、经皮肺穿刺活检确诊率及术后并发症的发生率。结果51例周围性肺部占位病变患者CT引导下经皮肺穿刺活检后穿刺成功率为100.00%。经皮肺穿刺活检确诊率为88.26%(45/51),包括27例恶性病变、18例良性病变,另外6例无法确诊病例者经手术证实为患肺癌为假阴性,假阴性率为11.76%。术后出血1例,疼痛2例,气胸3例,呼吸困难2例,并发症总发生率为15.69%。结论给予肺周围实性患者实行CT引导下经皮肺穿刺活检,其准确性与安全性较高,操作过程简单易行,对患者造成创伤极小,并发症发生率较低。Objective To explore the diagnostic value of CT-guided percutaneous lung biopsy in solid mass lesions around the lung.Methods A retrospective analysis of the clinical data of 51 patients with peripheral pulmonary space-occupying lesions admitted to our hospital from February 2018 to December 2019,and CT-guided percutaneous lung biopsy was performed to detect solid surrounding lung lesions.The success rate of puncture,the diagnosis rate of percutaneous lung biopsy and the incidence of postoperative complications were recorded.Results After CT-guided percutaneous lung biopsy,the puncture success rate of 51 patients with peripheral lung occupying lesions was 100.00%.The diagnosis rate of percutaneous lung biopsy was 88.26%(45/51),including 27 malignant lesions and 18 benign lesions.The other 6 undiagnosed cases were false-negative for lung cancer,and the false-negative rate was 11.76%.Postoperative bleeding occurred in 1 case,pain in 2 cases,pneumothorax in 3 cases,and dyspnea in 2 cases.The total incidence of complications was 15.69%.Conclusion CT-guided percutaneous lung biopsy for solid patients around the lungs is highly accurate and safe,the operation process is simple and easy,it causes minimal trauma to the patient,and the incidence of complications is low.
