关节镜下自体腘绳肌腱及Rigidfix横穿钉固定重建前交叉韧带的临床疗效

摘　　要：目的探讨关节镜下应用自体腘绳肌腱及Rigidfix横穿钉固定重建前交叉韧带的方法和疗效。方法回顾性分析2017年1月至2019年1月收治的前交叉韧带重建患者98例的临床资料,所有患者术前经MRI检查均为前交叉韧带断裂,均接受关节镜下的自体腘绳肌腱及Rigidfix横穿钉固定重建手术,并在治疗前和术后1年后对患者的患侧前交叉韧带进行Lysholm膝关节功能评分和国际膝关节文献委员会(IKDC)评分,对比治疗后患侧与健侧的膝关节活动程度。结果患者术后1年患侧前交叉韧带Lysholm评分[(92.62±2.94)分vs(39.54±3.36)分,t=77.323,P=0.000]、IKDC评分[(84.63±3.94)分vs(27.58±4.06)分,t=64.661,P=0.000]均显著高于术前,差异有统计学意义。术后1年,患侧膝关节活动度(屈曲度、伸直度)与健侧比较差异无统计学意义(P>0.05)。结论对前交叉韧带重建患者在关节镜下实施自体腘绳肌腱及Rigidfix横穿钉固定治疗法能够有效改善患侧膝关节功能,帮助其恢复前交叉韧带的形态和功能。Objective To observe the effect of arthroscopy reconstruction of anterior cruciate ligament(ACL)with autologous hamstring tendons and Rigidfix transfixion nails.Methods The clinical data of 98 patients with ACL reconstruction from January 2017 to January 2019 were retrospectively analyzed.All patients had ACL rupture by MRI before operation,and underwent arthroscopy reconstruction of autologous hamstring tendons and Rigidfix transfixion nails.The Lysholm knee function score and the International Knee Documentation Committee(IKDC)score of the ACL on the affected side were measured before and after treatment.The knee joint activity of the affected side and the healthy side were compared after treatment.Results After 1-year treatment,the Lysholm score[(92.62±2.94)points vs(39.54±3.36)points,t=77.323,P=0.000]and IKDC score[(84.63±3.94)points vs(27.58±4.06)points,t=64.661,P=0.000]of the affected side were significantly higher than those before treatment.After 1-year treatment,there was no statistical significance in the range of motion(flexion and extension)between the affected side and the healthy side(P>0.05).Conclusion Arthroscopy treatment of autogenous hamstring tendon and Rigidfix transfixion nails for patients with ACL reconstruction can effectively improve the condition of the affected knee joint and can help the ACL.
