不同石膏外固定方法对GartlandⅡ、Ⅲ型肱骨髁上骨折患儿肘关节近期功能和Baumann角的影响

摘　　要：目的分析不同石膏外固定方法对GartlandⅡ、Ⅲ型肱骨髁上骨折患儿肘关节近期功能和Baumann角的影响。方法对32例GartlandⅡ、Ⅲ型肱骨髁上骨折患儿进行闭合复位内固定后,随机将其分为观察组(16例,伸直130°石膏管型固定)和对照组(16例,伸直130°石膏托固定)。术后4周拆除石膏,术后6~8周拔除内固定。比较两组患儿术后1 d、4周、12周的Baumann角;比较两组患儿术后12周的肘关节屈伸活动度及提携角。结果 32例患儿均获得随访,时间为12周。术后4、12周,两组患儿的Baumann角比较,差异具有统计学意义(P<0.05)。术后12周,两组的肘关节屈伸活动度比较,差异无统计学意义(P>0.05);术后12周,两组的提携角比较,差异具有统计学意义(P<0.05)。结论石膏管型固定对于肱骨髁上骨折患儿术后Baumann角的维持和预防肘内翻发生具有一定作用,且效果优于石膏托固定。Objective To analyze the effects of different plaster external fixation methods on elbow joint function and Baumann angle in children with Gartland typeⅡandⅢsupracondylar fracture of humerus.Methods After closed reduction and internal fixation,32 children with GartlandⅡandⅢsupracondylar fracture of humerus were randomly divided into observation group(16 cases,plaster tube fixation under 130°extension)and control group(16 cases,plaster support fixation under 130°extension).The plaster was removed at 4 weeks after operation,and the internal fixation was removed at 6-8 weeks after operation.The Baumann angle was compared between the two groups at 1 d,4 weeks and 12 weeks after operation;the elbow flexion and extension activity and carrying angle at 12 weeks after operation were compared between the two groups.Results All 32 patients were followed up for 12 weeks.At 4 and 12 weeks after operation,the differences of Baumann angle between the two groups were statistically significant(P<0.05).At 12 weeks after operation,there was no significant difference in elbow flexion and extension activity between the two groups(P>0.05);at 12 weeks after operation,the difference in the carrying angle between the two groups was statistically significant(P<0.05).Conclusion Plaster tube fixation has a certain effect on maintaining Baumann angle and preventing cubitus varus in children with supracondylar fracture of humerus after operation,and the effect is better than plaster support fixation.
