罗哌卡因复合氢吗啡酮用于剖宫产术后硬膜外镇痛的效果及对应激反应、血流动力学的影响

摘　　要：目的探讨罗哌卡因复合氢吗啡酮用于剖宫产术后硬膜外镇痛的效果及对应激反应、血流动力学的影响。方法选取2018年9月至2019年4月收治的接受剖宫产手术的110例产妇作为研究对象,以随机数字法将其分为对照组和观察组,每组55例。对照组术后采用罗哌卡因复合吗啡硬膜外镇痛,观察组术后采用罗哌卡因复合氢吗啡酮硬膜外镇痛。比较两组的镇痛效果、应激反应指标及血流动力学指标。结果观察组术后6、12 h的VAS评分明显低于对照组(P<0.05)。观察组术后6、12 h的NE、Cor及AngⅡ水平均低于对照组(P<0.05)。观察组术后6、12 h的MAP、SpO2均高于对照组(P<0.05)。结论罗哌卡因复合氢吗啡酮用于剖宫产术后硬膜外镇痛中,能够有效提高镇痛效果,减轻应激反应,维持血流动力学稳定,值得临床推广。Objective To explore the effect of ropivacaine combined with hydromorphone on epidural analgesia after cesarean section and the influences on stress response and hemodynamics.Methods From September 2018 to April 2019,110 pregnant women undergoing cesarean section were selected as the study objects,and they were divided into control group and observation group by random number method,with 55 cases in each group.The control group was treated with ropivacaine and morphine epidural analgesia after operation,while the observation group was treated with ropivacaine and hydromorphone epidural analgesia after operation.The analgesic effect,stress response indexes and hemodynamic indexes of the two groups were compared.Results The VAS scores in the observation group were significantly lower than those in the control group at 6 and 12 h after operation(P<0.05).The levels of NE,Cor and AngⅡin the observation group were lower than those in the control group at 6 and 12 h after operation(P<0.05).MAP and SpO2 of the observation group were higher than those of the control group at 6 and 12 h after operation(P<0.05).Conclusion Ropivacaine combined with hydromorphone used in epidural analgesia after cesarean section can effectively improve the analgesic effect,reduce stress response and maintain hemodynamic stability,which is worthy of clinical promotion.
